综合管理模式下个性化氨氯地平服用方案在老年高血压中的应用

摘　　要：目的探究综合管理模式下个性化氨氯地平服用方案在老年高血压中的应用及效果。方法选取海南省人民医院2017年6月~2018年9月间收治的老年高血压患者132例,采用随机数字法将患者分为实验组和对照组,对照组采用常规老年高血压治疗方案,实验组采用综合管理模式下个性化氨氯地平服用方案。比较两组患者血压控制情况、治疗效果、生存质量评分及不良事件发生情况。结果治疗后两组患者血压均有所下降,治疗后第1、3、6及12个月实验组与对照组舒张压比较,差异均有统计学意义(P<0.05),治疗后3、6及12个月两组收缩压比较,差异均有统计学意义(P<0.05);实验组治疗有效率明显高于对照组,差异有统计学意义(P<0.05);实验组治疗后的社会功能、躯体功能、躯体疼痛、精神健康及生存质量总评分等均明显高于对照组,差异均有统计学意义(P<0.001);实验组患者不良事件发生率明显低于对照组,差异有统计学意义(P<0.05)。结论采用综合管理模式下个性化氨氯地平服用方案能够有效控制老年高血压,对改善患者生活质量及预后具有积极的意义,有较高临床推广价值。Objective To explore the effect of individualized amlodipine regimen in older patients with hypertension under comprehensive management mode.Methods 132 older patients with hypertension admitted to Hainan people's hospital from June 2017 to September 2018 were divided into the control group and the experimental group by random number method.The control group was treated with routine treatment for elderly hypertension,and the experimental group was treated with individualized amlodipine treatment under comprehensive management mode.The blood pressure,treatment effect,quality of life and the adverse events were documented.Results After treatment,the blood pressure of patients in two groups were decreased.There were significant differences in diastolic blood pressure between the two groups at 1,3,6 and 12 months after treatment(P<0.05).There were significant differences in systolic blood pressure between the two groups at 3,6 and 12 months after treatment(P<0.05).The response rate in the experimental group was significantly higher than that in the control group(P<0.05).The total scores of social function,physical function,physical pain,mental health and quality of life in the experimental group were significantly higher than those in the control group after treatment(P<0.001).The incidence of adverse events in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion Individualized amlodipine regimen under the comprehensive management mode could effectively control the hypertension in older patients,and improve the quality of life and prognosis of patients.
