基于实时超声弹性成像技术评价调经促孕针刺法对卵巢早衰的疗效研究

摘　　要：目的探讨实时超声弹性成像技术对调经促孕针刺法治疗卵巢早衰疗效的评估价值。方法选取2019年1月~2020年5月深圳市中医院针灸门诊及病房的60例卵巢早衰患者为研究对象。所有患者均接受调经促孕针刺法治疗,每周3次,连续治疗3个月经周期,对于闭经者,连续治疗3个自然月。观察患者治疗前后血清激素水平变化,并采用实时超声弹性成像技术检测患者治疗前后卵巢弹性模式和弹性应变率比值,分析各指标对调经促孕针刺法治疗卵巢早衰疗效的评估价值。结果治疗后患者FSH、LH水平低于治疗前,E2水平高于治疗前(P<0.05)。治疗后患者卵巢弹性模式评分、卵巢弹性应变率比值低于治疗前(P<0.05)。ROC曲线分析显示,卵巢弹性应变率比值、LSH、E2、LH、弹性模式的曲线下面积分别为0.829、0.752、0.741、0.722、0.709。结论调经促孕针刺法可明显改善卵巢早衰患者血清激素水平,促进卵巢功能恢复,同时实时超声弹性成像技术对调经促孕针刺法治疗卵巢早衰疗效的评估具有较高的应用价值。Objective To explore evaluation value of real-time ultrasonic elastography in the treatment of premature ovarian failure with acupuncture for regulating menstruation and promoting pregnancy.Methods 60 cases of premature ovarian failure in Shenzhen Hospital of traditional Chinese Medicine from January 2019 to May 2020 were selected as the research objects.All patients were treated with acupuncture for regulating menstruation and promoting pregnancy,3 times a week for 3 menstrual cycles,and 3 natural months for amenorrhea.The changes of serum hormone levels before and after treatment were observed,and the ovarian elastic mode and elastic strain rate ratio were detected by real-time ultrasonic elastography before and after treatment,and the evaluation value of each index for the treatment of premature ovarian failure by regulating menstruation and promoting pregnancy acupuncture was analyzed.Results After treatment,FSH and LH levels were lower than those before treatment,and E2 levels were higher than those before treatment(P<0.05).The score of ovarian elastic mode and the ratio of ovarian elastic strain rate were lower than those before treatment(P<0.05).ROC curve analysis showed that the area under the curve of ovarian elastic strain rate ratio,LSH,E2,LH and elastic mode were 0.829,0.752,0.741,0.722 and 0.709.Conclusion The acupuncture method of regulating menstruation and promoting pregnancy can obviously improve the serum hormone level of patients with premature ovarian failure and promote the recovery of ovarian function.At the same time,real-time ultrasonic elastography technology has high application value in the evaluation of the therapeutic effect of acupuncture for regulating menstruation and promoting pregnancy in the treatment of premature ovarian failure.
