中医综合疗法联合人绒毛膜促性腺激素治疗黄体功能不全性不孕症临床研究

摘　　要：目的:观察中医综合疗法联合人绒毛膜促性腺激素(HCG)治疗黄体功能不全性不孕症的临床疗效。方法:选择黄体功能不全性不孕症患者90例,按随机数字表法分为对照组和观察组各45例。对照组给予HCG肌注治疗,观察组在对照组基础上给予隔姜灸和补肾活血方治疗。观察2组黄体中期卵巢动脉血流指标及黄体功能变化。结果:观察组总有效率为95.56%,高于对照组71.11%,差异有统计学意义(P<0.05)。治疗前,2组黄体中期雌二醇(E2)、孕激素(P)、黄体生成素(LH)水平比较,差异均无统计学意义(P>0.05)。治疗后,2组黄体中期E2、P、LH水平较治疗前升高(P<0.05),且观察组黄体中期E2、P、LH水平高于对照组(P<0.05)。治疗前,2组卵巢动脉血流搏动指数(PI)、阻力指数(RI)及收缩期峰值流速(PSV)、舒张末期血流速度(EDV)比较,差异均无统计学意义(P>0.05)。治疗后,2组RI较治疗前降低(P<0.05),PSV、EDV较治疗前升高(P<0.05);且观察组RI低于对照组(P<0.05),PSV、EDV高于对照组(P<0.05)。结论:中医综合疗法可以改善黄体功能不全性不孕症患者卵巢动脉血流灌注,提高性激素水平,进而提高黄体功能,提高妊娠率。Objective:To observe the clinical effect of comprehensive therapy of Chinese medicine combined with human chorionic gonadotropin(HCG) for infertility caused by luteal phase defect. Methods: A total of 90 cases of patients with infertility caused by luteal phase defect were divided into the control group and the observation group according to the random number table method, 45 cases in each group. The control group was given intramuscular injection with HCG, and the observation group was additionally treated with moxibustion on ginger and Bushen Huoxue prescription based on the treatment of the control group. During the mid-luteal phase,the changes in blood flow indexes of ovarian artery and luteal function in the two groups were observed. Results:The total effective rate was 95.56% in the observation group,higher than that of 71.11% in the control group, the difference being significant(P<0.05). Before treatment, there was no significant difference being found in the comparison of the levels of estradiol(E2),progesterone(P) and luteinizing hormone(LH)during the mid-luteal phase between the two groups(P>0.05). After treatment,the levels of E2,P and LH during the midluteal phase in the two groups were increased when compared with those before treatment(P<0.05),and the levels in the observation group were higher than those in the control group(P<0.05). Before treatment,there was no significant difference being found in the comparison of pulsatility index(PI), resistance index(RI), peak systolic velocity(PSV) and end diastolic velocity(EDV) between the two groups(P>0.05). After treatment, the RI levels in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of PSV and EDV were increased(P<0.05);the RI level in the observation group was lower than that in the control group(P<0.05),and the levels of PSV and EDV were higher(P<0.05).Conclusion: Comprehensive therapy of Chinese medicine for patients with infertility caused by luteal phase defect can improve the blood perfusion of
