应用血管塞介入栓塞治疗儿童肺隔离症临床疗效观察

摘　　要：目的探讨应用血管塞介入栓塞异常供血动脉治疗儿童肺隔离症的临床疗效。方法将2015年-2018年入住郴州市第一人民医院的12例诊断为肺隔离症的患儿纳入研究对象,应用血管塞行异常供血动脉介入栓塞治疗。术后1月、3月、6月、1年随诊,观察肺部感染等临床症状改善情况,术后1年复查肺部CTA检查观察异常供血动脉闭塞情况及隔离肺组织吸收情况。结果12例病例均栓塞成功,无残余分流,术后无严重并发症,无反复肺部感染;术后1年复查肺部CTA显示血管塞远端血管及其分支血管完全闭塞,隔离肺组织明显萎缩,增强扫描无强化。结论应用血管塞介入栓塞异常供血动脉治疗儿童肺隔离症是一种安全、疗效显著的治疗方法。Objective To investigate the clinical effect of interventional embolization of abnormal feeding artery with vascular plug in treatment of children with pulmonary sequestration.Methods A total of 12 children were admitted in the study who were diagnosed with pulmonary sequestration in Chenzhou NO.1 People’s Hospital from 2015 to 2018,and they were treated by interventional embolization of abnormal feeding artery with vascular plug.The patients were followed up 1 month,3 months,6 months and 1 year after surgery to observe the improvement of pulmonary infection and other clinical symptoms,and the pulmonary CTA 1 year after surgery was performed to observe the occlusion of abnormal feeding artery and the absorption and dissipation of the isolated lung tissue.Results 12 cases were successfully embolized,and there was no residual shunt,serious complications,or recurrent lung infection occurred,and reexamination of pulmonary CTA 1 year after surgery showed complete occlusion of the distal vesselss.The isolated lung tissue was significantly atrophic,and there was no enhancement for enhanced scanning.Conclusion Interventional embolization of the abnormal feeding artery with vascular plug is a safe and effective treatment of pulmonary sequestration in children.
