双水平气道正压通气治疗慢阻肺伴呼吸衰竭患者的疗效分析

摘　　要：目的探讨双水平气道正压通气(BiPAP)治疗慢阻肺伴呼吸衰竭患者的疗效。方法选取2016年4月至2019年4月本院收治的82例慢阻肺伴呼吸衰竭患者为研究对象,采用蓝白双色球法分为两组,每组41例。对照组采用常规治疗,观察组在对照组基础上采用BiPAP通气治疗,比较两组治疗前后血气指标改善情况和治疗效果。结果治疗前,两组pH、PaO2、PaCO2指标比较差异无统计学意义;治疗后,观察组pH、PaO2指标高于对照组,PaCO2指标低于对照组(P<0.05);观察组总有效率(90.24%)高于对照组(73.17%),差异有统计学意义(P<0.05)。结论采用BiPAP治疗慢阻肺伴呼吸衰竭患者疗效显著,能有效改善患者血气指标,减轻患者临床症状,值得临床推广应用。Objective To explore the curative effect analysis of bilevel positive airway pressure(BiPAP)in the treatment of COPD patients with respiratory failure.Methods 82 patients with chronic obstructive pulmonary disease and respiratory failure admitted to our hospital from April 2016 to April 2019 were selected as the research subjects.They were divided into two groups by the blue and white double-color ball method,with 41 cases in each group.The control group was treated with conventional treatment,and the observation group was treated with BiPAP ventilation on the basis of control group.The improvement of blood gas indexes and the treatment effect were compared between the two groups before and after treatment.Results Before treatment,there was no significant difference in pH,PaO2,PaCO2 between the two groups;after treatment,the pH and PaO2 indicators of the observation group were higher than those of the control group,and the PaCO2 indicators were lower than the control group(P<0.05);the total effective rate of the observation group(90.24%)was higher than that of the control group(73.17%),and the difference was statistically significant(P<0.05).Conclusion BiPAP is effective in treating patients with chronic obstructive pulmonary disease and respiratory failure.It can effectively improve the blood gas index of the patients and reduce the clinical symptoms of the patients.It is worthy of clinical application.
