摘　　要：目的总结上尿路结石合并尿源性感染的诊治体会,提高对上尿路结石合并尿源性感染的诊治水平。方法回顾性分析本院泌尿外科在2016年12月至2019年12月收治的42例上尿路结石合并尿源性感染的临床资料,根据临床资料总结得出自己的诊治体会。结果42例上尿路结石合并尿源性感染的患者通过积极的抗感染治疗后,26例患者通过一期手术治疗成功,1例患者行输尿管镜碎石后出现尿源性脓毒血症,予加强抗感染、补液、支持治疗后痊愈出院。15例患者待炎症好转后行经尿道输尿管镜下D-J管置入术,二期手术治疗成功。结论上尿路结石合并尿源性感染只要足够重视,通过早期发现,积极抗感染治疗,及时手术解除梗阻,大多预后良好。Objective To improve the level of diagnosis and treatment of upper urinary tract calculi complicated with urinary infection by summarizing the diagnosis and treatment experience of upper urinary tract calculi complicated with urinary infection.Methods The clinical data of 42 cases of upper urinary tract calculi complicated with urinary infection treated in Department of Urology of our hospital from December 2016 to December 2019 were retrospectively analyzed,and the diagnosis and treatment experience was summarized according to the clinical data.Results Among the 42 patients with upper urinary tract calculi complicated with urinary infection,26 patients were successfully cured by one-stage operation after active anti infection treatment,one patient developed as urinary sepsis after ureteroscopic lithotripsy,and was cured and discharged after strengthening anti infection,fluid infusion,and supportive treatment.Fifteen patients underwent transurethral ureteroscopic D-J tube placement after the inflammation improved,and the second stage operation was successful.Conclusion If enough attention is paid to upper urinary calculi complicated with urinary infection,early detection,active anti infection treatment,and timely operation to remove the obstruction,can lead to good prognosis.
