舌下含服粉尘螨滴剂治疗单一和多重变应原变应性鼻炎的临床疗效观察

摘　　要：目的通过对比分析舌下含服粉尘螨滴剂治疗单一粉尘螨过敏和多重变应原过敏的变应性鼻炎两组数据,探讨多重变应原变应性鼻炎治疗的有效方法。方法回顾性分析自2012年4月至2018年5月,就诊我科的中重度变应性鼻炎,且均完成2年疗程的舌下特异性免疫治疗(SLIT)患者162例,其中单一粉尘螨致敏组(下称单一组)72例,多重致敏组(下称多重组,含2种上的变应原过敏)90例。比较分析两组患者治疗前、1年后、2年后,鼻部症状评分、鼻部总症状评分(TNSS)、药物评分(TMS)、有效率及Th1/Th2细胞所占百分比的变化。对相关数据进行统计学分析,对其疗效、依从性及不良反应进行综合评定。结果(1)两组患者鼻部症状(喷嚏、鼻痒、流涕、鼻塞)评分、TNSS、TMS与治疗前相比,差异有统计学意义(P<0.01)。(2)治疗1年后、2年后,两组患者鼻部症状、TNSS、TMS比较差异无统计学意义(P>0.05)。(3)单一组1年后有效率62.5%,明显优于多重组的53.33%;单一组2年后有效率70.83%,优于多重组的65.56%;但是经比较差异无统计学意义,两者疗程越长,疗效越接近。(4)治疗1年后、2年后,两组患者Th1细胞百分比含量明显升高,Th2细胞百分比含量明显降低,与治疗前比较差异有统计学意义(P<0.05);治疗1年后、2年后,两组Th1/Th2细胞所占百分比相接近,差异无统计学意义(P>0.05)。结论舌下含服粉尘螨滴剂对单一粉尘螨和多重变应原过敏的变应性鼻炎患者均有较好的疗效;且二者治疗疗程越长,其疗效越好,越接近;舌下含服粉尘螨滴剂是治疗多重变应原变应性鼻炎的有效方法,值得推广。Objective To explore an effective method for the treatment of multiple allergen allergic rhinitis by comparing and analyzing the data of sublingual dust mite drops in the treatment of single and multiple allergen allergic rhinitis.Methods From April,2012 to May,2018,162 patients with moderate to severe allergic rhinitis and completed 2-year course of sublingual specific immunotherapy(SLIT)were retrospectively analyzed.Among them,72 cases were enrolled in the single sensitization group(hereinafter referred to as the single group)and 90 cases were enrolled in the multiple allergens(hereinafter referred to as multi reorganization,including 2 or more allergens).The nasal symptom score,total nasal symptom score(TNSS),drug score(TMS),effective rate and percentage of Th1/Th2 cells were compared and analyzed before treatment,1 year and 2 years after treatment.The relevant data were statistically analyzed,and the efficacy,compliance and adverse reactions were comprehensively evaluated.Results(1)There were statistically significant differences in the scores of nasal symptoms(sneezing,itching,runny nose,stuffy nose),TNSS and TMS between the two groups before and after treatment(P<0.01).(2)After 1 year and 2 years of treatment,there was no significant difference in nasal symptoms,TNSS and TMS between the two groups(P>0.05).(3)After 1 year of treatment,the effective rate of the single group was 62.5%,which was significantly better than rate of 53.33%from the multiple recombination group.The effective rate of the single group after 2 years of treatment was 70.83%,which was better than rate of 65.56%from the multi recombination group,with no significant difference.The longer the course of treatment,the closer the curative effect was.(4)After 1 year and 2 years of treatment,the percentage of Th1 cells increased significantly,while the percentage of Th2 cells decreased significantly.Compared with that before treatment,the difference was statistically significant(P<0.05).After1 year and 2 years of.treatment,the percentages of Th1/
