摘  要:目的探讨血小板活化因子(PAF)、肿瘤坏死因子-α(TNF-α)、肺表面活性蛋白A(SP-A)在新生儿急性肺损伤中的表达及相关性。方法选取2018年9月~2019年9月在我院就诊的急性肺损伤新生儿70例设为急性肺损伤组,其中恢复期28例,急性期42例;存活63例,死亡7例。选取同一时期在我院就诊的肺炎新生儿50例设为肺炎组;另外选取在我院健康体检的新生儿50例设为对照组。采用酶联免疫吸附试验(ELISA)检测PAF、TNF-α水平,采用免疫斑点实验法进行SP-A检测,相关性采用Pearson分析。结果肺炎组和急性肺损伤组PAF、TNF-α表达水平高于对照组,SP-A水平低于对照组,差异均有统计学意义(P<0.05)。急性肺损伤组PAF、TNF-α表达水平高于肺炎组,SP-A水平低于肺炎组,差异均有统计学意义(P<0.05)。急性期PAF、TNF-α表达水平高于恢复期,SP-A水平低于恢复期,差异均有统计学意义(P<0.05)。死亡组PAF、TNF-α表达水平高于存活组,SP-A水平低于存活组,差异均有统计学意义(P<0.05)。PAF、TNF-α之间呈正相关(r=0.485,P=0.001);TNF-α、SP-A之间呈负相关(r=-0.358,P=0.002);PAF、SP-A之间呈负相关(r=-0.245,P=0.041)。结论PAF、TNF-α、SP-A在新生儿急性肺损伤中表达异常,三者之间有一定的相关性,参与新生儿急性肺损伤的发生,具有重要临床意义。Objective To study the expression and correlation of platelet activating factor(PAF),tumor necrosis factor-α(TNF-α),pulmonary surfactant protein A(SP-A)in neonatal acute lung injury.Methods In this paper,70 newborns with acute lung injury treated in our hospital from September 2018 to September 2019 were divided into acute lung injury group,including 28 in recovery period,42 in acute period,63 in survival period and 7 in death period.50 newborns with pneumonia treated in our hospital at the same period were divided into pneumonia group and 50 newborns examined in our hospital were divided into control group.The levels of PAF and TNF-αwere detected by enzyme-linked immunosorbent assay(ELISA),SP-A was detected by immune spot assay,and the correlation was analyzed by Pearson.Results The levels of PAF and TNF-αin pneumonia group and acute lung injury group were higher than those in the control group,and the levels of SP-A were lower than those in the control group(P<0.05).The expression level of PAF and TNF-αin acute lung injury group was higher than that in pneumonia group,and the level of SP-A was lower than that in pneumonia group(P<0.05).The expression level of PAF and TNF-αin acute stage was higher than that in convalescent stage,and the level of SP-A was lower than that in convalescent stage(P<0.05).The expression level of PAF and TNF-αin death group was higher than that in survival group,and the level of SP-A was lower than that in survival group(P<0.05).There was a positive correlation between PAF and TNF-α(r=0.485,P=0.001);there was a negative correlation between TNF-αand SP-A(r=0.358,P=0.002);there was a negative correlation between PAF and SP-A(r=0.245,P=0.041).Conclusion The expression of PAF,TNF-αand SP-A is abnormal in neonatal acute lung injury.There is a certain correlation between them.It is of great significance for the newborn to participate in the occurrence of neonatal acute lung injury.
