宫腔镜电切子宫内膜息肉对患者子宫内膜厚度、血清VEGF水平及月经量的影响

摘　　要：目的:分析宫腔镜电切术对子宫内膜息肉患者子宫内膜厚度、血清VEGF水平及月经量的影响。方法:选取本院2015年4月-2019年2月收治的100例子宫内膜息肉患者,随机数字表法分为两组,观察组采用宫腔镜电切术治疗,对照组采用宫腔镜下刮宫术治疗,术后随访12个月比较两组出血量、手术时间、住院时间、术后子宫内膜厚度、血管内皮生长因子(VEGF)、月经失血图(PBAC)评分以及复发和妊娠情况。结果:两组术中出血量、手术时间和住院时间比较无差异(P>0.05)。术后3、6、12个月随访,两组子宫内膜厚度、血清VEGF水平、PBAC评分均降低且观察组低于对照组(P<0.05)。术后12月内,观察组3例(5.9%)复发,30例(60.0%)妊娠;对照组10例(20.0%)复发,28例(56.5%)妊娠,观察组复发率低于对照组(P<0.05),妊娠率与对照组无差异(P>0.05)。结论:宫腔镜电切术治疗子宫内膜息肉效果更佳,对患者子宫内膜厚度、VEGF及PBAC评分改善程度优于宫腔镜下刮宫术,复发率更低。Objective:To analyze the effects of hysteroscopic electrotomy for treating women with endometrial polyps on their endometrial thickness,serum vascular endothelial growth factor(VEGF)level,and menstrual blood volume.Methods:100 women with endometrial polyps were selected and were divided into two groups according to the random number table method from April 2015 to September 2018.The women in the observation group were treated with hysteroscopic electrotomy,and the women in the control group were treated with hysteroscopic curettage.After 12 months of followed up,the bleeding volume,operation time,hospitalization time,endometrial thickness,serum VEGF level,pictorial blood loss assessment chart(PBAC)scores,and the rates of recurrence and pregnancy of women were compared between the two groups.Results:There were no significant differences in intraoperative blood loss,operative time,and hospital stay time of women between the two groups(P>0.05).After 3,6,and 12 months of followed up,the endometrial thickness,serum VEGF level,and PBAC score of women in both groups had all decreased significantly,and those of women in the observation group were significant lower than those of women in the control group(P<0.05).Within 12 months after operation,there were 3(5.9%)cases with recurrence and 30(60.0%)cases with pregnancy in the observation group,and there were 10(20.0%)cases with recurrence and 28(56.5%)cases with pregnancy in the control group.The recurrence rate of women in the observation group was significant lower than that of women in the control group(P<0.05).Conclusion:Hysteroscopic electrotomy for treating women with endometrial polyps has high effectiveness,which has higher improvement degree of endometrial thickness,VEGF level,and PBAC score of women when compared to those of hysteroscopic curettage,and has less recurrence rate.
