摘 要:目的评估自动调节持续气道正压通气法(auto-CPAP)联合呼吸方式训练治疗中重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的临床疗效。方法选取2019年4月—2020年8月本院收治的60例中重度OSAHS患者作为研究对象,采用随机数表法分为实验组和对照组两组,每组各30例。实验组患者采用auto-CPAP联合呼吸方式训练进行治疗,对照组患者采用单纯的auto-CPAP的方式进行治疗,对两组患者肺功能情况、主要疗效指标及睡眠结构的变化进行分析评价。结果实验组患者的肺功能得到显著改善,用力肺活量(FVC)、1秒用力呼出容积(FEV 1)及1秒用力呼气容积/用力肺活量(FEV 1/FVC)均高于对照组,差异均有统计学意义(均P<0.05);实验组患者的呼吸暂停低通气指数(AHI)、睡眠平均SpO 2、治疗有效压力及Epworth嗜睡评分(ESS)改善,差异均有统计学意义(均P<0.05);实验组患者睡眠结构得到改善,代表深睡眠的3期和快速眼动期(REM)期显著高于对照组,差异均有统计学意义(均P<0.05)。结论auto-CPAP联合呼吸方式训练治疗可以明显改善中重度OSAHS患者的临床症状。Objective To evaluate the clinical effect of Auto Continuous Positive Airway Pressure(AUTO-CPAP)combined with breathing mode training on patients with moderate to severe Obstructive Sleep Apnea Hypopnea Syndrome(OSAHS).Methods Sixty patients with moderate to severe OSAHS admitted to Jiangmen hospital affiliated to Sun Yat-sen University from April 2019 to August 2020 were randomly divided into experimental group and control group according to random number table method.Patients in the experimental group were treated with auto-CPAP combined respiratory training,while those in the control group were treated with auto-CPAP only,the changes of pulmonary function,main therapeutic indexes and sleep structure were analyzed and evaluated.Results The pulmonary function of OSAHS patients was significantly improved in the experimental group.The forced vital capacity(FVC),forced expiratory volume in the first second(FEV 1)and forced expiratory volume in the first second/forced vital capacity(FEV 1/FVC)were significantly higher in the experimental group than in the control group(P<0.05).The apnea-hypopnea index(AHI),average SpO 2 during sleep,the effective pressure and the Epworth Sleepiness Score(ESS)in the experimental group were all improved(P<0.05).The sleep structure of the patients in the experimental group was improved,which represented that the 3 stage of deep sleep and the rapid eye movement(REM)stage were significantly higher than those in the control group(all P<0.05).Conclusions The clinical symptoms of moderate and severe OSAHS could be significantly improved under the treatment of auto-CPAP combined with respiratory training.
