药物治疗管理服务在心血管疾病患者治疗中的研究

摘　　要：目的探讨专科临床药师为主多学科药师共同参与的以循证学为基础的专业药物治疗管理服务体系,旨在实现心血管疾病患者取得良好的治疗收益。方法选择2019年4月—2020年4月本院收治的高血压病患者100例、冠状动脉粥样硬化性心脏病患者100例、心力衰竭患者100例作为研究对象,采用抽取单双号分组法分为对照组和药物治疗管理干预组,每病种、每组各50例。3通过6个月的药物治疗管理服务体系与普通药学服务进行对比,对参与药物治疗管理体系的监护结果、药品不良反应、患者用药依从性,治疗转归情况进行统计分析。结果通过对此样本群的观察研究,推断该药物治疗管理服务体系组(干预组)患者的用药依从性,血脂、血压,再住院率,药源性损伤等监测指标与对照组存在显著差异,有临床推广意义。结论以专科临床药师为主,多学科临床药师共同参与的药物治疗管理服务体系,是实现个体化用药的先决条件,是实现合理用药的重要手段。可将其推广至神经系统疾病、血液系统疾病、老年性疾病、肿瘤的治疗,改善临床治疗结局,使更多患者受益。Objective The study explored the professional medication therapy management service system based on evidence-based medicine aiming to achieve good treatment benefits for patients with cardiovascular diseases,which was mainly developed by clinical pharmacists of cardiovascular and multi-disciplinary pharmacists.Methods 100 patients with hypertension,100 patients with coronary atherosclerotic cardiopathy,and 100 patients with heart failure those admitted to our hospital from April 2019 to April 2020 were enrolled as the study subjects.Patients with different disease were divided into two groups according the even and odd number,namely,the experiment group and the control group,50 cases in each group.After 6 month of application of medication therapy management service system and compared with the general pharmaceutical services,statistical method was used to analyzed monitoring results,adverse drug reactions and patients'compliance with medication as well as treatment outcomes in the medication therapy management service system.Results Then,results would have clinical promotion significance through observation and research based on the sample cluster,which suggested that monitoring indexes,namely,patient's medication compliance,blood lipids and blood pressure as well as re-hospitalization rate,drug-induced injury,etc.were significant different between the drug treatment management service system group and the control group.It presented clinical promoting significance.Conclusions The medication therapy management service system being mainly composed of specialist clinical pharmacists and multi-disciplinary clinical pharmacists,was a prerequisite for individualized medication and an important way to achieve rational drug use.Moreover,it could be extended to the treatment of nervous system diseases,blood system diseases and senile diseases as well as tumors,which would improve the clinical treatment outcome and benefit for more patients.
