宫颈癌患者外周血TSCM与Treg的分布特征及临床意义

摘　　要：目的探讨宫颈癌患者外周血中干细胞样记忆性T细胞(TSCM)与调节性T细胞(Treg)的分布特点及临床价值。方法选取2019年10月至2020年3月在徐州医科大学附属医院住院的宫颈癌、高级别上皮内瘤变、低级别上皮内瘤变各30例为观察组,同期15例宫颈炎患者作为对照组。采集外周静脉血2ml,流式细胞学方法检测TSCM与Treg的比例。结果宫颈癌组、高级别上皮内瘤变组、低级别上皮内瘤变组、宫颈炎组中TSCM表达呈显著上升趋势,Treg表达水平则逐渐下降(P<0.05);外周血中TSCM、Treg、Treg/TSCM的表达水平与临床分期、淋巴结转移和组织分化程度相关(P<0.05),而与患者年龄、病理类型、是否侵犯肌层及肿瘤大小等无明确相关。结论伴随肿瘤进展,机体抗肿瘤免疫储备能力逐渐减退,免疫抑制以及负调节能力逐步增强;基于TSCM和Treg的细胞免疫治疗未来有巨大的发展潜力。Objective:To investigate the expression of stem-like memory T cells(TSCM)and regulatory T cells(Treg)in peripheral blood of patients with uterine cervical cancer and their potential relationships with clinicopathological features and to explore its value of immunotherapy for cervical cancer.Methods:From October 2019 to March 2020,pretreated peripheral blood samples obtained from 105 patients hospitalized in the Affiliated Hospital of Xuzhou Medical University.The patients were divided into cervical cancer group,HSIL Group,LSIL Group and cervicitis group via their clinical pathology.TSCM and Treg were analyzed by flowcytometry.The relationships between these cells and the clinicopathological features were explored.Results:There was a remarkable increase of the expression of TSCM and a significance decrease of Treg and Treg/TSCM in cervical cancer Group,HSIL Group,LSIL Group and cervicitis group(P<0.05).The expressions of TSCM、Treg、Treg/TSCM were associated with clinical stages,differentiation degree and lymph node metastasis while it showed no significant correlation with the age,the size of tumor,pathological types and invasion depth.Conclusion:With the tumor progression,the immunocompetence of tumor-resistance has being weaken gradually while the immunosuppression shows the completely opposite trend.Moreover,the adoptive cellular immunotherapy of uterine cervical cancer based on the TSCM and Treg may have a significant clinical application value in the future.
