摘　　要：目的探讨不同支架管留置时间对行经尿道输尿管镜取石术(URL)的泌尿系结石患者远期排石效果及安全性的影响。方法选取2014年1月至2018年3月本院行URL的输尿管上段结石或肾结石患者共184例。按术后支架管留置时间差异分为两组,其中,术后支架管留置4周的96例患者为对照组;术后支架管留置2周的88例患者为观察组。比较两组拔管后4周的结石清除率、置管期间并发症发生率及止痛药应用率,结果两组拔管后4周的结石清除率比较,差异无统计学意义(P>0.05);观察组的置管期间并发症发生率显著低于对照组(P<0.05);两组止痛药应用率比较,差异无统计学意义(P>0.05)。结论行URL的泌尿系结石患者术后支架管留置2周可有效降低置管相关并发症发生率,且可获得与留置4周相近的排石效果,更具临床应用价值。Objective To explore the effect of different stent retention time on the long-term stone drainage effect and safety of patients with ureteroscopic lithotripsy(URL)urinary system.Methods A total of 184 patients who underwent URL for upper ureteral calculi or renal calculi were selected from January 2014 to March 2018.According to the difference of stent retention time,the patients were divided into two groups:96 patients with stent retention for 4 weeks as the control group and 88 patients with stent retention for 2 weeks as the control group.The stone removal rate,complication rate and analgesic application rate were compared between the two groups 4 weeks after extubation.Results There was no significant difference between the two groups in the complete stone removal rate 4 weeks after extubation(P>0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).At the same time,there was no significant difference in the application rate of painkillers between the two groups(P<0.05).Conclusions Two weeks of stent placement can effectively reduce the incidence of complications related to catheter placement in patients with URL urolithiasis,and the stone removal effect is similar to that of four weeks of stent placement.
