摘　　要：目的探索助产士对疤痕子宫产妇阴道分娩行会阴切开术的决策影响因素。方法采用目的抽样的方法,选取广州市3家医院的10名助产士进行半结构式访谈,采用内容分析法对访谈内容进行整理分析。结果共总结出除会阴切开指征以外的4个主题因素:助产士个人因素;产科医生的决策;产妇自身因素;医疗机构管理因素。结论需加强疤痕子宫孕妇的产前指导及医务人员助产专业知识培训,改善助产质量,以促进会阴切开术的合理应用,改善疤痕子宫产妇顺产后的生活质量。Objective To explore the factors that influence midwives’decision-making on performing episiotomy during vaginal birth for multiparas after cesarean.Methods Purposive sampling was used to recruit ten midwives from 3 hospitals in Guangzhou City for semi-structured interviews.The content analysis method was applied to analyze the interviews.Results Besides the medical indications of episiotomy,four themes of factors were concluded,including personal factors of midwives,decisions of obstetricians,maternal factors,management factors of medical institutions.Conclusions Prenatal education for multiparas with previous cesareans should be strengthened.Midwifery professional training for medical staff are required to improve the quality of midwifery.Appropriate performance of episiotomy should be promoted to help multiparas have better quality of postpartum life after vaginal birth.
