全髋关节置换术与人工股骨头置换术治疗老年股骨颈骨折的临床价值体会

摘　　要：目的研究老年股骨颈骨折患者在治疗过程中采用全髋关节置换术与人工股骨头置换术治疗方式的临床效果。方法选取2015年3月至2019年3月海门市第五人民医院收治的股骨颈骨折60例患者为本次实验研究对象,根据手术治疗方式的不同分成试验组和对照组,每组患者30例。对照组患者给予人工股骨头置换术,观察组患者实施全髋关节置换术,对比两组老年股骨颈骨折患者在治疗后的治疗效果。结果观察组患者在实施全髋关节置换术后统计的总体健康评分(74.61±3.12)分、躯体疼痛评分(75.81±3.29)分、生理职能评分(75.82±3.46)分、生理功能评分(76.79±2.91)分与活力评分(76.59±3.81)分,显著高于对照组总体健康评分(69.56±3.52)分、躯体疼痛评分(69.37±3.64)分、生理职能评分(69.30±3.17)分、生理功能评分(70.51±3.23)分与活力评分(68.26±4.12)分,观察组治疗总优良率93.33%显著高于对照组76.67%,差异有统计学意义(P<0.05)。结论在治疗老年股骨颈骨折疾病时选择全髋关节置换术实施治疗,对疾病的改善作用要显著高于人工股骨头置换术模式。Objective To study the effect of total hip arthroplasty and artificial femoral head replacement in the treatment of elderly patients with femoral neck fracture.Methods A total of 60 patients with femoral neck fracture treated in Haimen Fifth People's Hospital from March 2015 to March 2019 were selected as the experimental research objects.According to the different surgical treatment methods,they were divided into experimental group and control group,30 cases in each group.The control group was given artificial femoral head replacement,and the observation group was treated with total hip arthroplasty.The therapeutic effect of two groups of elderly patients with femoral neck fracture after treatment was compared.Results The total health score(74.61±3.12)score,body pain score(75.81±3.29)score,physiological function score(75.82±3.46)score,physiological function score(76.79±2.91)score and vitality score(76.59±3.81)score of patients in the observation group after total hip replacement were statistically analyzed,it was significantly higher than that of the control group in general health score(69.56±3.52)score,body pain score(69.37±3.64)score,physiological function score(69.30±3.17)score,physiological function score(70.51±3.23)score and vitality score(68.26±4.12)score,the total excellent and good rate of the observation group was 93.33%,which was significantly higher than 76.67%of the control group(P<0.05).Conclusion In the treatment of elderly patients with femoral neck fracture,total hip arthroplasty can improve the disease significantly better than that of artificial femoral head replacement.
