布托啡诺用于混合痔术后镇痛的效果观察

摘　　要：目的探讨布托啡诺用于混合痔术后镇痛的临床效果。方法选取2019年9月至2020年3月江苏省江阴市中医院收治的70例混合痔患者为研究对象,均在局麻下行外切内扎术,术后均给予自控静脉镇痛(PCIA),采用随机数字表法分为地佐辛组(D组)和布托啡诺组(B组),各35例。D组术毕PCIA配方:地佐辛20 mg+甲氧氯普胺20 mg,B组术毕PCIA配方:布托啡诺8 mg+甲氧氯普胺20 mg,两组均用生理盐水稀释至100 mL。比较术后6 h(T1)、术后12 h(T2)、术后24 h(T3)、术后48 h(T4)视觉疼痛模拟评分(VAS评分)和舒适度评分(BCS评分),观察恶心呕吐、头晕、口干、皮肤瘙痒、尿潴留等不良反应发生率。结果B组T1、T2、T3、T4 VAS评分均低于D组(P<0.05)。B组T1、T2、T3、T4 BCS评分均高于D组(P<0.05)。B组恶心呕吐发生率低于D组(P<0.05),其余不良反应发生率比较,差异无统计学意义(P>0.05)。结论布托啡诺PCIA可减轻混合痔患者的术后疼痛,提高术后舒适度,降低恶心呕吐不良反应发生率。Objective To investigate the clinical effect of butorphanol on postoperative analgesia of mixed hemorrhoids.Methods A total of 70 patients with mixed hemorrhoids in Jiangyin Hospital of Traditional Chinese Medicine from September 2019 to March 2020 were selected as the research objects.They were all treated with external incision and internal ligation under local anesthesia.All patients were given PCIA after operation.They were randomLy divided into dezocine group(Group D)and butorphanol group(Group B),35 cases in each group.The PCIA formula of Group D was dezocine 20 mg+metoclopramide 20 mg,and that of Group B was butorphanol 8 mg+metoclopramide 20 mg,both groups were diluted to 100 mL with normal saline.Postoperative 6 h(T1),postoperative 12 h(T2),postoperative 24 h(T3),postoperative 48 h(T4)VAS score and comfort score(BCS score)were compared.Adverse reactions such as nausea and vomiting,dizziness,dry mouth,pruritus,urinary retention were observed.Results The VAS scores of T1,T2,T3 and T4 in Group B were lower than those in Group D(P<0.05).The BCS scores of T1,T2,T3 and T4 in Group B were higher than those in Group D(P<0.05).The incidence of nausea and vomiting in Group B was lower than that in Group D(P<0.05),and there was no significant difference in other adverse reactions(P>0.05).Conclusion Butorphanol PCIA can relieve postoperative pain,improve postoperative comfort and reduce nausea and vomiting adverse reactions in patients with mixed hemorrhoids.
