升高血压及提高心率在显微精索静脉结扎术中的作用

摘　　要：目的探讨经腹股沟外环下切口显微镜下精索静脉结扎手术中,利用药物升高患者血压、并提高心率,加快术中动脉的分辨,缩短手术时间的可行性和安全性。方法回顾性分析2016年11月至2018年12月接受经腹股沟外环下切口显微镜下精索静脉结扎手术共97例患者的临床资料,其中2016年11月至2017年9月47例共55侧(A组)患者采用显微镜下精索静脉结扎手术;2017年10月至2018年12月50例共55侧(B组)采用同样术式,术中利用药物短暂升高血压、提高心率;统计两组间术中动脉分离时间、手术时间及动脉损伤的发生率。结果B组比A组手术时间及分离动脉时间短;所有患者中输精管动脉均无损伤;具有多条精索内动脉患者中,A组中动脉血管壁损伤5例,动脉误扎5例;B组中动脉血管壁损伤2例,误扎1例。结论在显微精索静脉结扎术中,利用药物升高血压并提高心率,加快对动脉系统的辨别,可缩短手术时间,并减少动脉的损伤率。Objective To explore the feasibility and safety of using of drugs to increase blood pressure and heart rate,speed up the identification of arteries and shorten the operation time in subinguinal microsurgical varicocelectomy.Methods A retrospective analysis was made of 97 cases of subinguinal microsurgical varicocelectomy from November 2016 to December 2018.From November 2016 to September 2017,55 sides of 47 castes(group A)underwent microsurgical varicocelectomy;from October 2017 to December 2018,55 sides of 50 cases(group B)underwent microsurgical varicocelectomy.During the operation,drugs were used to elevate blood pressure and heart rate.The time of arterial separation,operation time and the incidence of arterial injury were counted between the two groups.Results Group B had shorter operation time and arterial separation time than group A.Vas deferens artery was not damaged in all cases.In cases with multiple internal spermatic arteries.In group A,there were 5 cases of wall injury and 5 cases of artery misligation.In group B,there were 2 cases of middle artery injury and 1 case of artery misligation.Conclusions During microsurgical varicocelectomy,increasing blood pressure and heart rate can accelerate the identification of arterial system,shorten the operation time and reduce the rate of arterial injury.
