改良床边盲插鼻肠管在内科ICU危重患者中的应用效果

摘　　要：目的探讨内科ICU危重患者床边盲插鼻肠管的改良方法及应用效果。方法选取漳州市医院内科ICU需要置入鼻肠管进行肠内营养的危重病患者103例,采用随机数字法分为观察组(n=52)和对照组(n=51),对照组采用床边盲插鼻肠管的常规方法,观察组采用改良床边盲插鼻肠管操作方法。比较两组患者置管成功率、置管时间、麻醉药物使用率、置管费用。结果观察组与对照组的置管成功率分别为98.08%、90.20%,差异无统计学意义(P>0.05);观察组置管时间短于对照组,麻醉药物使用率(11.54%)低于对照组(72.55%),置管费用少于对照组,差异均具有统计学意义(P<0.05)。结论在内科ICU危重患者中应用改良床边盲插鼻肠管可缩短置管时间,减少麻醉药物使用率,降低治疗成本。Objective To explore the improved method and application effect of blind insertion of naso-intestinal tube by bedside of critically ill patients in internal medicine ICU.Methods A total of 103 critically ill patients in the internal medicine ICU of Zhangzhou Hospital who need to be placed in a naso-intestinal tube for enteral nutrition were selected and divided into observation group(n=52)and control group(n=51)by random number method.The control group was bedside blind.The conventional method of naso-intestinal tube insertion,the observation group adopted a modified bedside blind insertion method.The success rate of catheterization,catheterization time,usage rate of anesthetics,and catheterization cost were compared between the two groups.Results The success rate of catheter placement in the observation group and the control group was 98.08%and 90.20%,respectively,and the difference was no statistically significant(P>0.05);the catheter placement time in the observation group was shorter than that of the control group,and the use rate of anesthetics(11.54%)was lower than that of the control group(72.55%),the cost of catheter placement was less than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The application of modified bedside blind insertion of nasal and intestinal tubes in critically ill patients in internal medicine ICU can shorten the catheterization time,reduce the use of anesthetics,and reduce the treatment cost.
