硝普钠联合多巴胺治疗急性左心衰竭患者的临床疗效

摘　　要：目的探讨硝普钠联合多巴胺治疗急性左心衰竭患者的临床疗效。方法选取2017年12月至2018年12月医院收治的110例急性左心衰竭患者,随机分为两组,各55例。两组患者均接受常规对症综合治疗,研究组在此基础上加用硝普钠和多巴胺治疗。比较治疗后的心率(HR)、呼吸频率、平均动脉压(MAP)、24 h尿量;复查超声心动图,检测左心室射血分数(LVEF)、每分输出量(CO)、心脏指数(CI)、每搏输出量(SV);复查血B型脑利钠肽前体(NT-proBNP)水平,并进行6 min步行距离试验(6MWT)。结果治疗后,研究组患者的LVEF、CO、CI、SV、HR、呼吸频率、MAP、6MWT、24 h尿量、NT-proBNP水平明显优于对照组(P<0.05)。结论硝普钠联合多巴胺治疗急性左心衰竭患者能够进一步改善其左心功能,改善患者的生命体征,降低NT-proBNP水平,提高劳动耐力。Objective To investigate the clinical effect of nitroprusside combined with dopamine in the treatment of acute left heart failure.Methods From December 2017 to December 2018,110 patients with acute left heart failure were randomly divided into two groups,with 55 patients in each group.Both groups received conventional symptomatic comprehensive treatment,the control group was limited to this,the study group on the basis of its plus nitroprusside and dopamine treatment.Heart rate(HR),respiratory rate,mean arterial pressure(MAP),and urine volume were recorded after treatment.Reexamination of echocardiography,detection of left ventricular ejection fraction(LVEF),output per minute(CO),cardiac index(CI),output per stroke(SV).Blood level of type B brain natriuretic peptide precursor(NT-proBNP)was re-examined and 6 min walking test(6MWT)was performed.Results After treatment,the LVEF,CO,CI,SV,HR,respiratory rate,MAP,6MWT,24-hour urine output,and NT-proBNP levels of the study group were significantly better than those of the control group(P<0.05).Conclusion Acute left heart failure with nitroprusside sodium and dopamine treatment,can further improve the left heart function,improve the patient's vital signs,reduce the level of NT-proBNP,improve labor endurance.
