摘 要:目的探讨sCD163和触珠蛋白在结核性胸腔积液中的表达水平变化及其临床意义。方法使用随机数字表法随机选取2016年1月至2018年12月河北大学附属医院收治的结核性胸腔积液患者40例作为观察组,选择同期收治入院的非结核性胸腔积液患者(均为变应性胸膜炎患者)40例作为对照组。两组患者均进行胸腔穿刺取胸腔积液,采用酶联免疫吸附测定法检测胸腔积液中的sCD163及触珠蛋白的表达水平并进行组间比较。结果观察组胸腔积液中sCD163和触珠蛋白水平均高于对照组(t=12.166、20.528,P<0.05)。sCD163诊断结核性胸腔积液的约登指数约为0.794,而触珠蛋白约为0.694,sCD163具有更高的诊断价值(P<0.01);二者联合应用诊断结核性胸腔积液的约登指数约为0.839,明显高于单独使用sCD163或触珠蛋白诊断。结论结核性胸腔积液中sCD163及触珠蛋白表达水平明显增高,二者联合应用对结核性胸腔积液具有更高的临床诊断价值。Objective To investigate the changes of sCD163 and haptoglobin expression in tuberculous pleural effusion and its clinical significance.Method Forty patients with tuberculous pleural effusion admitted to Affiliated Hospital of Hebei University from January 2016 to December 2018 were randomly selected as the study subjects.40 patients with non-tuberculous pleural effusion admitted to the hospital were selected as the control group.All patients included in the study were treated with thoracic puncture for thoracic puncture and pleural effusion.The supernatant was centrifuged and placed in a refrigerator at-80℃for use.ELISA was used to detect sCD163 and haptoglobin levels in pleural effusions of the two groups.The expression levels of sCD163 and haptoglobin,as well as the diagnostic value of sCD163 and haptoglobin and tuberculous pleural effusion.Result The levels of sCD163 and haptoglobin in the observation group were higher than those in the control group(t=12.166,20.528,P<0.05).The sCD163 Yoden index(YI)is about 0.794,the haptoglobin Yoden index(YI)is about 0.694,and sCD163 has a higher diagnostic value(P<0.01);but the combined diagnosis of the Yoden index(YI)is about 0.839,significantly higher diagnostic value than sCD163 or haptoglobin alone.Conclusion The expression of sCD163 and haptoglobin in pleural effusion of patients with tuberculous pleural effusion increased significantly.The combination of sCD163 and haptoglobin for the diagnosis of tuberculous pleural effusion has higher clinical diagnostic value.
