左炔诺孕酮宫内节育系统联合米非司酮治疗围绝经期功能失调性子宫出血临床研究

摘　　要：目的探讨左炔诺孕酮宫内节育系统(LNG-IUS)联合米非司酮治疗围绝经期功能失调性子宫出血(简称功血)的临床疗效及对患者月经、激素水平的影响。方法选取黄石市第五医院2018年收治的围绝经期功血患者158例,按入院编号的单双数分为治疗组和对照组,各79例。两组患者均予常规治疗,并在刮宫术5 d后于阴道处放置LNG-IUS,治疗组患者加服米非司酮片,连服3个月。结果治疗组总有效率为97.47%,显著高于对照组的89.87%(P<0.05);治疗后,治疗组患者子宫内膜厚度、月经量均显著小于对照组,卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)水平均显著低于对照组(P<0.05);治疗组与对照组不良反应发生率相当(10.13%比6.33%,χ2=0.754,P>0.05)。结论在LNG-IUS基础上联合米非司酮治疗围绝经期功血,可调节月经量及激素水平。Objective To investigate the clinical efficacy of levonorgestrel intrauterine system(LNG-IUS)combined with mifepristone in the treatment of perimenopausal dysfunctional uterine bleeding and its effect on menstruation and hormonal levels.Methods Totally 158 patients with perimenopausal dysfunctional uterine bleeding admitted to Huangshi Fifth Hospital in 2018 were selected and divided into the treatment group and the control group according to the odd and even admission number,79 cases in each group.The patients in the two groups were given routine treatment and the LNG-IUS was placed in the vagina 5 d after curettage,on this basis,the patients in the treatment group were treated with Mifepristone Tablets for 3 months.Results The total effective rate of the treatment group was97.47%,which was significantly higher than 89.87%of the control group(P<0.05).After treatment,the endometrial thickness and menstrual volume of patients in the treatment group were significantly lower than those in the control group,and the levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E2)in the treatment group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the treatment group was similar to that in the control group(10.13%vs.6.33%,χ2=0.754,P>0.05).Conclusion LNG-IUS combined with mifepristone in the treatment of perimenopausal dysfunctional uterine bleeding can regulate the menstrual volume and hormone levels of patients.
