旋体动椎法治疗L1~4腰椎后关节错缝症40例临床观察

摘　　要：目的观察旋体动椎法治疗L1~4腰椎后关节错缝症的临床疗效。方法将80例L1~4腰椎后关节错缝症患者随机分为观察组与对照组各40例。两组均给予基础手法,在此基础上,观察组在第2次治疗时配以旋体动椎法,对照组在第2次治疗时配以斜扳法。两组患者的治疗次数均为2次,每日1次。观察两组的视觉模拟量表(VAS)评分、Owestry功能障碍指数评分、患椎(相对下位椎体)旋转的棘突点距离差和总有效率。结果两组治疗后VAS评分、Owestry功能障碍指数评分和患椎(相对下位椎体)旋转的棘突点距离差均下降,且治疗组低于对照组,差异有统计学意义(P<0.05);观察组临床总有效率为97.50%,对照组为90.00%,组间比较差异无统计学意义(P>0.05)。结论旋体动椎法能有效缓解腰痛,改善腰椎功能,减小患椎(相对下位椎体)旋转的棘突点距离差,提高患者的生活质量。Objective:To observe the clinical effect on treatment of lumbar posterior joint dislocation syndrome with L1~4 by Rotating Affected Vertebra′s Technique by Rotating Torso.Methods:A total of 80 patients were randomly divided into the observation group and control group,40 cases in each group.Both groups were given a basic manipulation.On this basis,the observation group was equipped with Rotating Affected Vertebra’s Technique by Rotating Torso at the second treatment,and the control group was equipped with Oblique Pulling Technique at the second treatment.Both groups were treated twice,once a day.The changes in the VAS score,ODI score,the distance difference between the affected vertebra′s spinous process point and the lower vertebra′s spinous process point,and the total efficiency were observed.Results:After treatment,the VAS score,ODI score,and the distance difference between the affected vertebra′s spinous process point and the lower vertebra′s spinous process point decreased,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05);the total efficiency was 97.50% in the observation group and 90.00% in the control group,and the difference was not statistically significant(P>0.05).Conclusion:Rotating Affected Vertebra′s Technique by Rotating Torso can effectively alleviate low back pain,improve lumbar spine function,reduce the distance difference between the affected vertebra′s spinous process point and the lower vertebra′s spinous process point,and improve the quality of life of patients.
