摘　　要：目的观察侧卧位与截石位分娩对初产妇会阴裂伤程度和产后出血的影响。方法选取2018年1月至2019年12月入住医院的250例拟自然分娩的初产妇作为研究对象,按照产妇年龄、孕周、体质量指数等基本资料匹配分为观察组和对照组,每组125例,观察组采用侧卧位分娩、对照组采用截石位分娩,比较两组产妇分娩后会阴裂伤程度及产后出血情况。结果观察组第二产程时间较对照组缩短(P<0.05);观察组会阴裂伤程度较对照组更轻(P<0.05);观察组两组产妇产后2 h出血量、产后出血发生率比较差异均无统计学意义(P>0.05)。结论初产妇行侧卧位分娩的第二产程时间更短,对会阴裂伤程度影响更小,产后出血量与截石位分娩相当,并未增加产后出血风险。Objective To observe the effect of lateral position and lithotomy position on the degree of perineal laceration and postpartum hemorrhage of primiparas.Methods To select 250 cases of primiparas who were expected to give birth naturally and were admitted to the hospital from January 2018 to December 2019 as the study objects.According to the basic data such as maternal age,gestational age,body mass index,they were divided into an observation group and a control group,with 125 cases in each group.The observation group was delivered in the lateral position,and the control group was delivered in the lithotomy position to compare the degree of perineal laceration and postpartum hemorrhage between the two groups.Results The second stage of labor duration in the observation group was shorter than that in the control group(P<0.05).The degree of perineal laceration in the observation group was lighter than that in the control group(P<0.05).There was no statistical significance in the comparison of the bleeding amount at 2 hours after delivery and the incidence of postpartum hemorrhage in the two groups(P>0.05).Conclusion The second stage of labor duration for primiparas who gave birth in a lateral position was shorter and had less effect on the degree of perineal laceration.The amount of postpartum hemorrhage was equivalent to that of delivery at the lithotomy position,and it did not increase the risk of postpartum hemorrhage.
