超声、CT、X线平片诊断结肠肿瘤性肠梗阻的临床价值比较

摘　　要：目的:分析结肠肿瘤性肠梗阻的影像特点,比较超声、CT(DSCT)、腹部x线平片这三种影像检查技术在诊断结肠肿瘤性肠梗阻方面的优劣性。方法:回顾性收集本院2017年1月—2020年4月间经临床证实为结肠肿瘤性肠梗阻患者50例,所有结肠肿瘤手术前均行腹部B超、CT及x线平片检查,对三种检查方法的诊断结果进行统计分析。结果:CT对结肠肿瘤性肠梗阻的诊断符合率(98.26%)明显高于超声(61.37%)(P<0.01)及x线平片检查(52.18%)(P<O.01),CT对梗阻部位的诊断正确率(97.35%)也优于超声(75.28%)(P<0.01)及x线平片检查(63.55%)(P<O.01)。结论:CT在诊断结肠肿瘤性肠梗阻方面明显优于超声及x线平片检查,因为它能进行三维后处理重建,从而全面显示结肠肿瘤及肠梗阻的部位,值得在临床中广泛推广应用。Objective:To analyze the imaging features of colonic neoplastic intestinal obstruction,and compare the advantages and disadvantages of three imaging techniques of ultrasound,dual energy CT(DSCT)and abdominal X-ray plain film in the diagnosis of colonic neoplastic intestinal obstruction.Methods:Fifty patients with colon tumor intestinal obstruction confirmed clinically in our hospital from January 2017 to April 2020 were retrospectively collected.All colon tumors were examined by abdominal B-ultrasonography,dual energy CT and X-ray plain film before operation,and the diagnostic results of the three methods were statistically analyzed.Results:The diagnostic coincidence rate of dual energy CT for colon tumor intestinal obstruction(98.26%)was significantly higher than that of ultrasound(61.37%)(P<0.01)and X-ray plain film examination(52.18%)(P<0.01).The diagnostic accuracy rate of dual energy CT for obstructive site(97.35%)was also better than that of ultrasound(75.28%)(P<0.01)and X-ray plain film examination(63.55%)(P<0.01).Conclusion:Dual energy CT is obviously superior to ultrasound and X-ray plain film in the diagnosis of colon tumor intestinal obstruction,because it can perform three-dimensional post-processing reconstruction,thus comprehensively displaying the location of colon tumor and intestinal obstruction,which is worthy of wide application in clinical practice.
