穴位敷贴治疗腹泻型肠易激综合征临床选穴规律数据挖掘研究

摘　　要：目的采用数据挖掘方法分析穴位敷贴治疗腹泻型肠易激综合征的选穴特点和规律,为临床相关治疗提供参考。方法计算机检索中国知识资源总库(CNKI)、中文科技期刊数据库(VIP)、万方数据知识服务平台(Wanfang Data)、PubMed、Medline、Embase自建库至2019年10月收录的穴位敷贴治疗腹泻型肠易激综合征的临床文献并进行筛选,采用频次统计、聚类分析、关联规则方法分析穴位敷贴治疗腹泻型肠易激综合征的选穴规律。结果共纳入文献71篇,穴位敷贴治疗腹泻型肠易激综合征的主要腧穴为神阙、天枢、脾俞、足三里、大肠俞,任脉、足阳明胃经和足太阳膀胱经腧穴使用频次最高,所选腧穴主要集中在胸腹部和腰背部,分析高频选穴得出23条关联规则,聚类分析将高频选穴聚成4类。结论穴位敷贴治疗腹泻型肠易激综合征以运脾化湿为主,辅以温补元阳、调畅腑气,选穴规律符合其病机特点,可为临床治疗提供参考。Objective To analyze characteristics of acupoint selection and laws of acupoint application in the treatment of diarrhea-predominant irritable bowel syndrome(IBS-D);To provide reference for the clinical treatment.Methods Clinical literature about acupoint application in the treatment of IBS-D were retrieved from CNKI,VIP,Wanfang Data,PubMed,Medline,and Embase from establishment to October 2019.The acupoint selection laws of acupoint application in the treatment of IBS-D were analyzed by using frequency statistics,cluster analysis,and correlation rule analysis.Results Totally 71 related articles were obtained.After analysis,it was found that the main acupoints in acupoint application in the treatment of IBS-D were Shenque(RN8),Tianshu(ST25),Pishu(BL20),Zusanli(ST36),and Dachangshu(BL25);the most commonly used acupoints were in Ren Meridian,Foot-yangming Stomach Meridian and Foot-taiyang Bladder Meridian;the selected acupoints were mainly distributed on the chest and abdomen,and back.Through analyzing high-frequency acupoints,23 association rules were obtained,and high-frequency acupoints could be classified to four groups by cluster analysis.Conclusion Acupoint application in the treatment of IBS-D is mainly to activate spleen to dissipate dampness,supplemented with warmly tonifying yang and regulating qi.Characteristics of acupoint selection is accordance with the features of etiology and pathogenesis of the disease,which can provide a reference for the clinical treatment of IBS-D in clinic.
