摘　　要：目的采用典则相关分析法(CCA)分析冠状动脉粥样硬化性心脏病(CHD)患者脉图递归定量分析(RQA)特征与凝血功能的相关性,探寻可反映CHD患者凝血功能的脉图RQA特征参数。方法纳入2018年7月-2019年11月上海中西医结合医院及上海中医药大学附属曙光医院心内科住院CHD患者205例,采用压力脉冲信息分析系统(Smart TCM-Ⅰ)采集脉图,提取脉图RQA特征,检测患者凝血酶原时间(PT)、活化部分凝血酶时间(APTT)、国际标准化比率(INR)及纤维蛋白原(FIB),采用CCA分析脉图RQA特征与凝血四项的相关性。结果 2组变量间的第一对典则变量的相关系数为0.608,超过2组指标间的任一简单相关系数,差异有统计学意义(P<0.05),表明2组变量有一定相关性;脉图RQA特征递归率(RR)和最长竖直/水平线段长度(V_(max))在第一对典则变量上的载荷系数分别为0.904、0.816,在第一对典则变量上的交叉载荷系数分别为0.550、0.496;凝血指标APTT、INR、FIB在第一对典则变量上的载荷系数分别为0.792、-0.935、0.828,在第一对典则变量上的交叉载荷系数分别为0.481、-0.568、0.503。结论 APTT、FIB与脉图RQA特征RR、V_(max)呈正相关,INR与RR、V_(max)呈负相关,脉图RQA特征(RR、V_(max))能在一定程度上反映APTT、INR、FIB水平。Objective To analyze the correlation between recursive quantitavityw analysis(RQA)characteristics of diagram pulse and coagulation function in patients with coronary atherosclerotic heart disease(CHD)by canonical correlation analysis(CCA);To explore the RQA characteristic parameters of pulse diagram that can reflect the coagulation function of patients with CHD.Methods Totally 205 patients with CHD in Shanghai Integrated Traditional Chinese and Western Medicine Hospital and Shuguang Hospital of Shanghai University of Traditional Chinese Medicine from July,2018 to November,2019 were enrolled.All parameters of pulse wave were collected using the pressure pulse information analysis system(Smart TCM-Ⅰ).The RQA characteristics of patients with CHD were extracted.Four coagulation parameters were collected:prothrombin time(PT),activated partial thromboplastin time(APTT),international normalized ratio(INR)and fibrinogen(FIB).Correlation between RQA characteristics and four coagulation items of CHD patients was analyzed by CCA.Results The correlation coefficient of the first canonical variable between the two groups was 0.608,which was larger than any simple correlation coefficient between the two groups with statistical significance(P<0.05),indicating that the two groups of variables had certain correlation.The load coefficients of RQA characteristics RR and Vmax on the first canonical variable were 0.904 and 0.816,and the cross load coefficients were 0.550 and 0.496,respectively;the load coefficients of APTT,INR and FIB on the first canonical variable were 0.792,-0.935 and 0.828,respectively,and the cross load coefficients were 0.481,-0.568 and 0.503,respectively.Conclusion APTT and FIB were positively correlated with RR and Vmax,and INR was negatively correlated with RR and Vmax.suggesting RQA characteristics(RR,Vmax)of pulse diagram could reflect the level of coagulation indexes(APTT,INR and FIB)to a certain extent.
