弯角椎体成形术对骨质疏松性椎体压缩骨折老年患者治疗效果及骨水泥填充率的影响

摘　　要：目的探讨弯角椎体成形术(percutaneous corner vertebroplasty,PCVP)对骨质疏松性椎体压缩骨折(osteoporotic vertebral compression fracture,OVCF)老年患者治疗效果及骨水泥填充率的影响。方法选取我院OVCF老年患者86例,分为观察组(n=43)与对照组(n=43)。对照组采取经皮椎体成形术(percutaneous vertebroplasty,PVP),观察组采取PCVP。统计两组术前及术后临床疗效。结果(1)术后6个月两组ODI及VAS分值观察组低于对照组(P<0.05);(2)术后6个月两组伤椎前缘高度较术前增高、伤椎Cobb角较术前缩小(P<0.05),且观察组伤椎前缘高度高于对照组、伤椎Cobb角小于对照组(P<0.05);(3)观察组骨水泥填充优良率高于对照组(95.35%vs.81.40%)(P<0.05);(4)观察组并发症发生率低于对照组(9.30%vs.25.58%,P<0.05)。结论采取PCVP治疗OVCF老年患者,可有效减轻其疼痛程度,增大伤椎前缘高度、缩小伤椎Cobb角,促使腰椎功能恢复,且骨水泥填充优良率较高、并发症发生率低,具有安全性。Objective To explore the effect of PCVP on the treatment effect and bone cement filling rate in elderly patients with OVCF.Methods A total of 86 elderly patients with OVCF in our hospital were selected and divided into study group(n=43)and control group(n=43).The control group took PVP and the study group took PCVP.The clinical efficacy of the two groups before and after operation were calculated.Results(1)The scores of ODI and VAS of the two groups in the study group were lower than those in the control group at 6 months after operation(P<0.05);(2)the height of the anterior edge of the injured vertebrae in the two groups was higher than that before operation at 6 months after operation.The Cobb angle was smaller than that before oper⁃ation(P<0.05),and the height of the front edge of the injured vertebrae in the study group was higher than that of the control group,and the Cobb angle of the injured vertebrae was smaller than that of the control group(P<0.05);(3)the excellent rate of bone cement filling in the study group was higher than the control group(95.35%vs.81.40%)(P<0.05);(4)the complication rate in the study group was lower than the control group(9.30%vs.25.58%)(P<0.05).Conclusion The use of PCVP in the treatment of elderly patients with OVCF can effective⁃ly reduce their pain,increase the height of the anterior edge of the injured vertebra,reduce the Cobb angle of the injured vertebra,and promote the recovery of lumbar spine function safely.
