单孔胸腔镜手术切口缝合技术改进与效果评价

摘　　要：目的探讨单孔胸腔镜手术切口缝合及引流管固定技术改进的安全性及可行性。方法回顾性分析2019年1-12月江南大学附属医院胸心外科收治的80例患者的临床资料,根据切口缝合技术不同分为观察组和对照组,比较两组患者切口愈合情况、术后疼痛评分、术后1个月疤痕评分及患者满意度。结果两组患者在切口感染率、二次缝合率、引流外渗率方面差异无统计学意义。观察组术后VAS疼痛评分及BCS舒适度评分优于对照组(P=0.022、0.041),观察组拆线时间短于对照组(P<0.001)。观察组术后1个月各项疤痕评分均优于对照组(P<0.001),且患者满意度高于对照组(P=0.026)。结论单孔胸腔镜手术利用可吸收线连续缝合切口安全可行,实现了微创与美学的有机结合,利于患者术后康复并提高患者满意度。Objective To investigate the safety and feasibility of improved technique of incision suture and drainage tube fixation in single⁃port video⁃assisted thoracoscopic surgery.Methods We retrospectively analyzed the clinical data of 80 patients who underwent single⁃port video⁃assisted thoracoscopic surgery from January 2019 to December 2019 in our hospital.According to different incision suture techniques,we divided them into observation group and control group.Results There were no statistically significant differences in wound infection rate,re⁃sewing rate and pleural fluid outflow rate between the two groups.The VAS and BCS of the observation group were better than that of the control group(P=0.022,0.041),and the suture removal time of the observation group was shorter than that of the control group(P<0.001).All scar scores(VSS,POSAS,SCAR)in the observation group were better than those in the control group one month after the surgery(all P<0.001),and patient satisfaction rate was higher than the control group(P=0.026).Conclusion The continuous incision suture with absorbable thread is safe and feasible for single⁃port video⁃assisted thoracoscopic surgery,which realizes the organic combination of minimally invasive and aesthetics,and is conducive to the postoperative rehabilitation and the improvement of patient satisfaction.
