摘  要:目的探讨人参总皂苷(TSPG)对骨关节炎大鼠关节疼痛和软骨修复的作用。方法将50只SD雌性大鼠随机分为5组,人参总皂苷低浓度组(TSPG-L 125μg/ml)、人参总皂苷中浓度组(TSPG-M 250μg/ml)、人参总皂苷高浓度组(TSPG-H 500μg/ml)、模型组和空白对照组。采用双侧膝关节注射碘乙酸法建立骨关节炎大鼠模型,人参总皂苷给药采用关节腔注射法。检测各组大鼠压痛阂值和热痛阈值。干预完成后,取每组双侧膝关节进行组织病理观察。结果OA造模成功,造模4周后,压热痛结果显示:高中低浓度人参总皂苷组的压热痛阈值相较于模型组,均有显著提升(P<0.01);病理结果显示:人参总皂苷组软骨细胞和软骨基质接近正常水平,有肥大软骨细胞少量,与模型组相比OARSI评分显著下降(P<0.01);软骨细胞CCK实验结果显示:与模型组相比,人参总皂苷组软骨细胞活力显著提高(P<0.01)。结论人参总皂苷对改善关节疼痛、修复受损软骨疗效显著,其机制可能与抑制炎症因子,减少软骨细胞凋亡和基质损伤,促进软骨细胞修复有关。Objective To observe the effect of ginseng total saponins on cartilage repair in rats with osteoarthritis.Methods Eighty SD female rats were randomly divided into 5 groups,including high concentration group of total ginsenoside(TSPG-H 500μg/ml),medium concentration group of total ginsenoside(TSPG-M 250μg/ml),low concentration group of total ginsenoside(TSPG-L 125μg/ml),model group and blank control group.The osteoarthritis of rat was induced by injecting iodoacetic acid into both knee joints,and the total saponins of ginseng were also administered by joint cavity injection.The tenderness threshold and thermal pain threshold of each group were detected.After the intervention,the bilateral knee joints of each group were taken for histopathological observation.Results The reaction of OA model was successful.4 weeks after mold-building,the tenderness pain results showed that the high-medium-low-concentration ginseng total saponins group had a significant increase in the tenderness threshold compared with the model group(P<0.01);pathological results showed:ginseng total saponins group chondrocytes The cartilage matrix was close to the normal level,and there were a small amount of hypertrophic chondrocytes.Compared with the model group,the OARSI score was significantly reduced(P<0.01);the chondrocyte CCK test results showed that:compared with the model group,the activity of chondrocytes in the ginseng total saponins group was significantly improved(P<0.01).Conclusion Ginsenosides have a significant effect on improving joint pain and repairing damaged cartilage.The mechanism is related to its ability to inhibit inflammatory factors,reduce chondrocyte apoptosis and matrix damageand,and promote the repair of chondrocytes.
