P450arom PON1 PRL联合RDW早期诊断子宫内膜异位症的应用价值

摘　　要：目的探讨细胞色素P450芳香化酶(P450arom)、对氧碎酶-1(PON1)、催乳素(PRL)结合红细胞分布宽度(RDW)检测对子宫内膜异位症的早期诊断临床意义。方法2018年1月至2019年5月子宫内膜异位症患者168例,轻度患者(Ⅰ~Ⅱ)87例,重度患者(Ⅲ)81例,另选取健康体检患者168例作为对照组,比较两组不同严重程度患者RDW、P450arom、PON1、PRL水平,分析疾病严重程度与RDW、P450arom、PON1、PRL水平的相关性,比较单独检测和联合检测间的差异。结果观察组患者RDW、P450arom、PRL水平高于对照组,PON1低于对照组;重度组患者RDW、p450arom、PRL水平高于对照组,PON1低于对照组,患者疾病严重程度与RDW、P450arom、PRL水平呈正相关,与PON1呈负相关,通过ROC曲线分析,联合检测灵敏度高于单独检测,且轻度患者RDW、P450arom、PON1、PRL水平的临界值分别为33.26μg/L,0.44U,72.01 KU/L,20.09%,重度患者RDW、P450arom、PON1、PRL水平的临界值分别为52.09 ng/ml,0.61 U,42.01 KU/L,26.66%。结论RDW.P450arom、PON1、PRL水平的联合检测可显著提升子宫内膜异位症患者的诊断效能,建议临床推广。Objective To study the clinical significance of P450arom,PON1 and PRL combined with RDW in early diagnosis of endometriosis.Methods 168 patients with endometriosis admitted to our hospital from January 2018 to May 2019 were selected as the study subjects,87 patients with mild endometriosis(Ⅰ-Ⅱ)and 81 patients with severe endometriosis(Ⅲ).168 patients with health examination in our hospital were selected as the control group.The RDW,P450 AROM and other parameters of patients with different severity were compared between the two groups.The differences of PON1 and PRL levels were analyzed.The correlation between disease severity and RDW,P450arom,PON1 and PRL levels was analyzed,and the difference of efficacy between single and joint detection was compared.Results The levels of RDW,P450 AROM and PRL in the observation group were significandy higher than those in the control group,and PON1 was significandy lower than those in the control group;the levels of RDW,P450 AROM and PRL in the severe group were significandy higher than those in the control group,and PONl was significantly lower than those in the control group.The severity of the disease was positively correlated with the levels of RDW,P450 AROM and PRL,and negadvely correlated with PON1.Through ROC curve analysis,the sensitivity of combined detection was significandy higher than that of single detection,and the critical values of RDW,P450arom,PON1 and PRL levels in mild patients were 33.26 μg/L,0.44 U,72.01 KU/L and 20.09%,respectively.The critical values of RDW,P450arom,PON1 and PRL levels in severe patients were 52.09 ng/ml,0.61 U,42.01 KU/L and 26.66%respectively.Conclusion The combined detection of RDW,P450arom,PON1 and PRL levels can significantly improve the diagnostic efficacy of endometriosis patients,and it is recommended to be popularized in clinic.
