摘  要:目的探讨髋部骨折患者骨标本中H型血管、骨密度的变化。方法收集临床髋部骨折患者的年龄、体重指数、血常规及生化指标等信息,按照骨密度T值将患者分为3组,包括骨密度正常组、骨量减低组、骨质疏松症组,分析各组间骨密度的变化。收集各组患者术中的骨标本,按照免疫荧光染色的流程处理,探讨骨组织中H型血管的存在及变化情况,相关数据进行统计学分析。结果3组患者的一般资料包括血常规、生化、电解质及凝血指标,对应数据组间差异无统计学意义(P>0.05)。骨密度正常组、骨量减低组及骨质疏松症组成骨代谢指标Ⅰ型原胶原氨基端延长肽(procollagenⅠN terminal peptide,PINP)分别为(40.70±5.64)、(58.99±6.23)、(88.97±7.41)ng/mL,组间差异有统计学意义(P<0.05)。3组间破骨代谢指标Ⅰ型胶原C端肽β降解产物(beta carboxy-terminal collagen cross links,β-CTX)分别为(518.66±24.19)、(609.26±39.68)、(771.76±52.89)pg/mL,组间差异有统计学意义(P<0.05)。3组间腰椎骨密度值分别为(1.33±0.34)、(0.98±0.12)、(0.56±0.08)g/cm 2,组间差异有统计学意义(P<0.05)。3组间股骨颈骨密度值分别为(1.22±0.23)、(0.88±0.10)、(0.42±0.06)g/cm 2,组间差异具有统计学意义(P<0.05)。3组骨标本中H型血管分别为(25.30±1.95)%、(15.56±1.26)%、(3.36±0.62)%,组间差异有统计学意义(P<0.05)。结论髋部骨折患者的骨内存在H型血管,其量随骨密度的降低而明显减少,为髋部骨折的防治提供了一定的实验基础。Objective To explore the relationship of bone mineral density(BMD)and type H vessel in bone from the patients with hip fracture.Methods Information from patients such as ages,BMI,blood routine examination,and liver and kidney functions was collected.All patients were divided into three groups of normal bone mineral density,decreased bone mineral density,and osteoporotic group.The bone mineral density changes were measured by dual energy X-ray absorptiometry(DEXA).Human bone samples were collected during surgery and processed by immunostaining in order to discuss the changes of type H vessels in bone.All experimental data were collected for statistical analysis by single factor Analysis of Variance(ANOVA).Results Routine blood tests,biochemistry and electrolytic markers,and coagulation index in three groups had no significant difference(P>0.05).The values of PINP were(40.70±5.64),(58.99±6.23),and(88.97±7.41)ng/mL,respectively(P<0.05).The levels ofβ-CTX was(518.66±24.19),(609.26±39.68),and(771.76±52.89)pg/mL,respectively(P<0.05).The values of lumbar BMD were(1.33±0.34),(0.98±0.12),and(0.56±0.08)g/cm 2,respectively(P<0.05).The values of femoral neck BMD were(1.22±0.23),(0.88±0.10),and(0.42±0.06)g/cm 2,respectively(P<0.05).The type H vessels in human bone were detected by immunofluorescence staining.The values of type H vessels were(25.30±1.95)%,(15.56±1.26)%,and(3.36±0.62)%in the three groups respectively(P<0.05).Conclusion There are type H vessels in bone from patients with hip fractures.The number of the type H vessels decrease with reduce of bone mineral density.The relationship of the bone mineral density and type H vessels in this study provide a certain experimental basis for the diagnosis and treatment of hip fracture.
