摘 要:目的探讨生长分化因子5(growth differentiation factor 5,GDF5)单核苷酸多态性与退变性腰椎滑脱症椎间盘真空征的相关性。方法选择2017年1月~2018年1月于张家口市第二医院收治的206例退变性腰椎滑脱症患者为研究对象,根据CT检查是否存在椎间盘真空征将患者分为真空征组(39例)和非真空征组(167例)。所有患者均采集外周静脉血,采用TaqMan探针SNP基因分型技术进行GDF5基因rs143383位点的基因多态性检测。结果本组椎间盘真空征的发生率为18.93%(39/206),真空征组和非真空征组、不同性别间的GDF5基因rs143383位点基因频率分布、等位基因频率差异显著(P<0.05)。Logistic回归分析显示,GDF5基因多态性与退变性腰椎滑脱症患者发生椎间盘真空征独立相关,携带TT基因型个体发生椎间盘真空征的风险明显增高(OR=1.725,95%CI:1.261-4.990,P=0.000)。结论携带GDF5 rs143383位点TT基因型的退变性腰椎滑脱症患者发生椎间盘真空征易感性明显增加。Objective To investigate the correlation between single nucleotide polymorphism of growth differentiation factor 5(GDF5)and vacuum sign of intervertebral disc in degenerative lumbar spondylolisthesis.Methods From January 2017 to January 2018,206 cases of degenerative lumbar spondylolisthesis confirmed by CT in the Second Hospital of Zhangjiakou were selected in this research.According to whether there was vacuum sign of intervertebral disc in CT examination,the patients were divided into vacuum sign group(39 cases)and non-vacuum sign group(167 cases).Peripheral venous blood samples were collected from all patients.Single nucleotide polymorphism(SNP)genotyping with TaqMan probe was used to detect the genetic polymorphism at rs143383 locus of GDF5 gene.Results The incidence rate of intervertebral disc vacuum sign was 18.93%(39/206).There were significant differences in the frequency distribution and allele frequency of rs143383 locus of GDF5 gene between the vacuum sign group and the non-vacuum sign group and different genders(P<0.05).Logistic regression analysis showed that GDF5 gene polymorphism was independently associated with intervertebral disc vacuum sign in patients with degenerative lumbar spondylolisthesis.The risk of intervertebral disc vacuum sign in individuals with TT genotype increased significantly(OR=1.725,95%CI:1.261-4.990,P=0.000).Conclusion The susceptibility of degenerative lumbar spondylolisthesis patients carrying TT genotype of GDF5 rs143383 to intervertebral disc vacuum sign is significantly increased.
