摘  要:目的初步探讨老年冠心病患者肠道菌群结构的特点及差异。方法选取2018年10月~2020年1月北京某三甲医院住院的老年男性患者66例,将血管狭窄≥50%的36例作为冠心病组,血管狭窄<30%的30例作为对照组,收集患者大便进行处理,采用16S rDNA高通量测序方法分析样本肠道菌群结构。结果Alpha多样性分析显示,冠心病组操作分类单元和Shannon指数高于对照组[(295.70±64.33)类vs(242.10±69.11)类,P=0.037;3.25±0.54 vs 2.33±0.56,P=0.020],Simpson指数低于对照组(0.08±0.03 vs 0.12±0.03,P=0.019);门层级水平,2组样本多样性无明显差异(P>0.05);冠心病组放线菌门细菌丰度较对照组增多、梭杆菌门细菌丰度较对照组减少(P<0.05,P<0.01),变形菌门较对照组减少,近统计学差异(P=0.0527)。属层级水平,2组多样性和物种丰度均有差异(P<0.05),冠心病组以双歧杆菌属和真杆菌属为主(P<0.05),对照组以志贺菌属和粪杆菌属丰度较高(P<0.05)。结论冠心病组肠道菌群的多样性更高,放线菌门、双歧杆菌属和真杆菌属相对丰度更高。Objective To study the characteristics of intestinal flora structure in elderly CHD patients.Methods Of the 66 elderly male patients admitted to our hospital from October 2018 to January 2020,36 with their vascular stenosis≥50% served as a CHD group and 30 with their vascular stenosis<30% served as a control group.Their stool was processed and the structure of intestinal flora was analyzed by 16S rDNA high-throughput sequencing.Results Alpha diversity analysis showed that the operational taxonomic unit and Shannon index were higher while the Simpson index was lower in CHD group than in control group(295.70±64.33 vs 242.10±69.11,P=0.037;3.25±0.54 vs 2.33±0.56,P=0.020;0.08±0.03 vs 0.12±0.03,P=0.019).No significant difference was detected in sample diversity at the gate level between the two groups(P>0.05).The species abundance of Actinobacteria was higher and that of Clostridiobacteria and Fusobacteria was lower in CHD group than in control group(P<0.05,P<0.01).A significant difference was detected in diversity and species abundance at the genus level between the two groups(P<0.05).Bifidobacterium and Eubacterium were dominant in CHD group(P<0.05)while Shigella and Faecalibactria were dominant in control group(P<0.05).The abundance of Shigella and fecal bacillus was higher in control group than in CHD group(P<0.05).Conclusion The diversity of intestinal flora and the abundance of Actinobacteria,Bifidobacterium and Eubacterium are higher in CHD patients than in controls.
