血清因子与老年慢性心力衰竭患者心功能的相关性分析

摘　　要：目的探究血清因子与老年慢性心力衰竭(CHF)患者心功能的相关性。方法选取2019年1月~2020年1月本院老年病科收治的CHF患者180例为CHF组,CHF组心功能分级:Ⅱ级57例,Ⅲ级84例,Ⅳ级39例。另选同期体检的健康志愿者50例为对照组,检测受试者血清TNF-α、C反应蛋白(CRP)及白细胞介素8(IL-8)水平,监测LVEF、左心室舒张末期内径(LVEDD)和左心室收缩末期内径(LVESD),采用Pearson相关分析。结果CHF组血清TNF-α、CRP、IL-8、LVEDD及LVESD明显高于对照组(P<0.05),LVEF明显低于对照组[(25.70±9.08)%vs(49.43±4.12)%,P<0.05]。心功能Ⅱ级、Ⅲ级、Ⅳ级患者血清TNF-α、CRP、IL-8、LVEDD和LVESD呈明显升高趋势,LVEF呈明显降低趋势(P<0.05)。CHF患者血清TNF-α、CRP和IL-8水平与LVEF呈负相关(r=-0.551,P=0.004;r=-0.613,P=0.017;r=-0.652,P=0.031),而与LVEDD和LVESD呈正相关(P<0.05,P<0.01)。结论血清TNF-α、CRP和IL-8水平与CHF患者心功能存在显著的相关性,有可能作为老年CHF患者病情严重程度和预后的评估指标。Objective To study the relationship between serum factors(TNF-a,CRP and IL-8)and cardiac function in elderly CHF patients.Methods One hundred and eighty CHF patients admitted to our hospital from January 2019 to January 2020 served as a CHF group and 50 healthy volunteers undergoing physical examination in our hospital during the same period served as a control group.There were 57 cases with grade Ⅲ cardial function,84 cases with grade Ⅲ,39 cases with grade Ⅳ in CHF group.Their serum TNF-α,CRP,IL-8 levels and LVEF,LVEDD,LVESD were measured and analysized by Pearson correlation analysis.Results The serum TNF-α,CRP and IL-8 levels were significantly higher,the LVEDD and LVESD were significantly longer(P<0.05)while the LVEF was significantly lower(25.70%±9.08%vs 49.43%±4.12%,P<0.05)in CHF group than in control group.The serum TNF-α,CRP,IL-8 levels,LVEDD and LVESD increased significanthy while LVEF decreased in patients with cardiac function grade Ⅱ,Ⅲ and Ⅳ(P<0.05).Pearson correlation analysis showed that serum TNF-α,CRP and IL-8 levels were negatively related with LVEF(r=-0.551,P=0.004;r=-0.613,P=0.017;r=-0.652,P=0.031)and positively related with LVEDD and LVESD in CHF patients(P<0.05,P<0.01).Conclusion Serum TNF-α,CRP and IL-8 levels are closely related with cardiac function in elderly CHF patients and can thus be used as an index in assessing the severity and outcome of CHF.
