含服自制蜂蜜绿茶液防治头颈部肿瘤患者放射性口腔黏膜炎的效果观察

摘　　要：目的对由头颈部肿瘤放疗所致的放射性口腔黏膜炎(RIOM)采用自制蜂蜜绿茶液含服法进行防治的效果展开观察。方法观察对象为2019年2月至12月期间医治的、经细胞学(或病理组织学)确诊的112例头颈部肿瘤患者,经随机数字表法各归入对照组(56例)、蜂蜜绿茶组(56例)。在放射干预初始,对照组行常规护理,蜂蜜绿茶组除了常规护理,还增加以下处理:早、中、晚、放疗前15 min、放疗后15 min与0.5 h含服20~30 ml自制蜂蜜绿茶液,先口腔中循环3 min上下,再缓慢咽下。对比分析2组放疗期间RIOM发生状况、疼痛分级、重度RIOM发生率与发生时间。结果在RIOM发生率方面,对比对照组,蜂蜜绿茶组偏低;在重度RIOM发生率,对照组、蜂蜜绿茶组依次是45%、16%,2组差异有统计学意义(P<0.05)。在口腔内重度疼痛发生率对照组、蜂蜜绿茶组各为69%、39%,2组差异有统计学意义(P<0.05)。结论放疗时,头颈部肿瘤患者含服自制蜂蜜绿茶液,能够使自身口腔湿润度提高,使炎症受抑,帮助口腔受损黏膜尽早修复,使重度RIOM发生率大幅下降,推迟RIOM的发生,减轻患者疼痛程度,使其临床症状得到缓解,以及生活品质得到提升,对头颈部肿瘤放疗所致RIOM具备有效预防作用。Objective To investigate the effects of oral self-made honey and green tea solution therapy on the prevention and treatment of radiotherapy-induced oral mucositis(RIOM)in patients with head and neck tumors.Methods A total of 112 patients with head and neck tumors diagnosed by cytology or histopathology between February 2019 and December 2019 were included as the subjects in the study.All patients were randomly divided into the control group and honey+green tea group(n=56 each).At the beginning of radiotherapy,the control group was given routine care alone.In addition,the honey+green tea group received 20~30 ml oral self-made honey and green tea solution three times daily and at 15 min before and after radiotherapy(rinse the mouth for 3 min before swallowing slowly).The occurrence of RIOM,pain grade,incidence of severe RIOM and time to RIOM during radiotherapy in the two groups were compared and analyzed.Results Compared with the control group,the incidence of RIOM was lower than that in the honey+green tea group.The incidence of severe RIOM in the control group and honey+green tea group was 45%and 16%,respectively,with significant difference(P<0.05).The oral severe pain grade in the control group and honey+green tea group was 68%and 39%,respectively,with significant difference(P<0.05).Conclusion During radiotherapy,oral self-made honey and green tea solution therapy may increase oral moisturization,inhibit inflammation,repair damaged oral mucosa early,greatly reduce the incidence of severe RIOM,delay the occurrence of RIOM,reduce the pain degree,relieve the clinical symptoms,and improve the quality of life,which iseffective in the prevention of RIOM in patients with head and neck tumors.
