摘  要:目的探讨COX-2和α-Synuclein在子宫内膜异位症合并不孕症患者子宫内膜中的表达差异及相关性。方法选取2016年6月—2017年8月于湖南省妇幼保健院就诊并接受治疗的54例子宫内膜异位症合并不孕症患者作为观察组,其中增生期和分泌期患者分别为28和26例。选取同期于湖南省妇幼保健院因子宫颈病变但不合并子宫内膜异位症而接受子宫切除术的54例患者作为对照组,其中增生期和分泌期患者各27例。分离并收集患者的在位、异位以及正常子宫内膜组织。采用免疫组织化学法检测内膜组织中COX-2和α-Synuclein的组织原位表达与分布;采用逆转录聚合酶链反应和Western blotting检测内膜组织中COX-2和α-Synuclein的mRNA和蛋白表达;采用Spearman法分析COX-2和α-Synuclein表达的相关性。结果观察组COX-2和α-Synuclein阳性细胞数较对照组高(P<0.05)。EMT在位内膜组、EMT异位内膜组α-Synuclein mRNA的相对表达量较对照组高(P<0.05)。EMT在位内膜组、EMT异位内膜组分泌期和增殖期COX-2和α-Synuclein mRNA的相对表达量较对照组高(P<0.05),且同组的分泌期均较增殖期高。EMT在位内膜组、EMT异位内膜组COX-2和α-Synuclein蛋白相对表达量较对照组高(P<0.05)。观察组分泌期和增殖期COX-2、α-Synuclein蛋白相对表达量较对照组高,且同组的分泌期均较增殖期高。经Spearman相关性分析,EMT患者异位内膜组织中COX-2与α-Synuclein的表达呈正相关(rs=0.885,P<0.05)。结论COX-2和α-Synuclein在子宫内膜异位症患者的内膜组织中异常过表达且呈正相关性,提示两者协同过表达可能促进子宫内膜异位症的发生、发展,进而导致患者不孕,其有望成为该疾病诊断和基因治疗的重要靶点。Objective To investigate the expression and correlation of COX-2 and α -Synuclein inendometrial tissues of patients with endometriosis and infertility. Methods From June 2016 to August 2017, 54patients with endometriosis and infertility who were treated in our hospital were included in the observation group,28 patients and 26 patients in the proliferative and secretory phases. 54 patients without endometriosis whounderwent hysterectomy at the same time as cervical lesions in our hospital were included in the control group,including 27 patients in the proliferative and 27 patients in secretory phases. The in-situ, ectopic, and normalendometrial tissues of the above subjects were stripped and collected. The expression and distribution of COX-2 and α -Synuclein in the samples were detected by immunohistochemistry. The RNA and protein expressions of COX-2and α -Synuclein were detected by RT-RCR and Western blotting. The Spearman method was used to analyzecorrelation between COX-2 and α-Synuclein expression. Results Cox-2 and α-Synuclein in the observation groupincreased significantly in the proliferative and secretory membrane tissues compared with the control group, withstatistically significant differences (P < 0.05). The expressions of COX-2 and α-Synuclein in the endometrial tissuesof the two groups were significantly higher in the secretory stage than in the proliferative stage (P < 0.05). Inaddition, the expression of COX-2 in the ectopic endometrial tissues of the observation group was positivelycorrelated with the expression of l-synuclein (rs = 0.885, P < 0.05). Conclusions COX-2 and α -Synuclein areabnormally overexpressed in the endometrial tissue of patients with endometriosis, have positive correlation witheach other, suggesting that the synergistic overexpression of the two may promote the development of endometriosisand lead to infertility, in which they play an important role, and is expected to become an important target for thediagnosis and gene therapy of this disease.
