加速康复外科围手术期护理对腹腔镜下卵巢良性肿瘤切除术的干预效果分析

摘　　要：目的研究加速康复外科(ERAS)围手术期护理对腹腔镜下卵巢肿瘤切除术患者的干预效果。方法以2017年3月至2019年12月于华中科技大学协和深圳医院接受手术治疗的86例卵巢良性肿瘤患者为研究对象。抛硬币法分成试验组及对照组,每组各43例。对照组实施常规围手术期护理干预,试验组实施ERAS围手术期护理干预。比较两组患者的术后卧床时间、排气时间、并发症发生情况、住院时间与费用,以及干预前后心理状态变化、手术前后应激反应情况。结果试验组患者术后卧床时间、排气时间及住院时间均短于对照组,住院费用少于对照组(均P<0.05);试验组术后压疮发生率低于对照组(P<0.05);干预后试验组SAS、SDS评分低于对照组(P<0.05);术后第3天试验组肾上腺素、皮质醇水平均高于对照组(P<0.05)。结论ERAS围手术期护理可有效促进腹腔镜下卵巢肿瘤切除术患者的术后康复,降低压疮发生风险,有效缓解患者的焦虑、抑郁等负性情绪,减轻应激反应程度。Objective To study the effect of perioperative nursing of accelerated rehabilitation surgery(ERAS)in patients with laparoscopic benign ovarian cyst.Methods From March 2017 to December 2019,86 patients with benign ovarian cyst who underwent laparoscopic surgery in Huazhong University of Science and Technology Union Shenzhen were included and divided into experimental group and control group randomly,with 43 cases in each group.Routine perioperative nursing intervention was performed in control group.While the patients in experimental group received ERAS perioperative nursing intervention.Bed rest time,exhaust time,occurrence of complications,length of hospital stay psychological changes,stress response and medical cost were compared between the two groups.Results The bed time,exhaust time and length of hospital stay in the experimental group were shorter than those of the control group.And the hospital expenses were less than those of the control group(all P<0.05).There was no significant difference between the experimental group and the control group in the incidence of incision infection,urinary tract infection and respiratory tract infection(all P>0.05).The incidence of pressure ulcers in experimental group was lower than that of the control group(P<0.05).After intervention,SAS and SDS scores of the experimental group were lower than those of the control group(all P<0.05).The levels of adrenaline and cortisol in the experimental group were higher than those in the control group(P<0.05).Conclusions Perioperative nursing of ERAS can effectively promote the postoperative recovery of patients with benign ovarian cyst under laparoscopy,reduce the risk of pressure ulcers,effectively relieve patients'negative emotions including anxiety and depression,and reduce the degree of stress response.
