摘　　要：目的探讨“T”形桡骨远端锁定钢板结合克氏针内固定治疗NeerⅡ型锁骨远端骨折的疗效和安全性。方法回顾性分析2016年3月至2019年3月采用“T”形桡骨远端锁定钢板结合克氏针内固定治疗25例NeerⅡ型锁骨远端骨折患者的临床资料。患者年龄21~70岁;右侧13例,左侧12例。受伤至手术时间2~7 d。结果本组25例均获得随访,随访时间平均(10.34±2.55)个月。切口均一期愈合,未发生切口感染、钢板或螺钉断裂以及螺钉退出等并发症。术后骨折均愈合,骨折愈合时间平均(10.25±2.44)周。末次随访时肩关节功能Constant-Murley评分平均92.8(80~97)分,其中优19例,良4例,可2例,优良率为92%。结论“T”形桡骨远端锁定钢板结合克氏针内固定治疗NeerⅡ型锁骨远端骨折疗效确切,术后患者可早期进行功能锻炼,有利于骨折愈合及肩关节功能恢复,值得在临床推广。Objective To observe the clinical efficacy and safety of internal fixation with T-shaped distal radius locking plate combined with kirschner wire in the treatment of distal clavicle fracture with Neer typeⅡ.Methods The clinical data of 25 distal clavicle fracture patients with Neer typeⅡunderwent internal fixation of T-shaped distal radial locking plate combined with kirschner wire from March 2016 to March 2019 were analyzed retrospectively.There were 15 males and 10 females,with aged from 21 to 70 years old.The fractures located on the right side in 13 cases and the left side in 12 cases.The time from injury to operation was 2 to 7 days.The fracture healing and complications were observed after operation,and the shoulder joint function was evaluated by Constant-Murley score.Results All patients were followed up for(10.34±2.55)months averagely.All fractures achieved healing during(10.25±2.44)weeks averagely without complications as wound infection,broken of plate or screw and screw exit.At the last follow-up,the Constant-Murley score of shoulder joint function was 80 to 97 with an average of 92.8 points.The outcomes revealed excellence in 9 cases,good in 4 cases,fair in 2 cases,and the fineness rate was 92%.Conclusions The surgery of internal fixation with T-shaped distal radius locking plate combined with kirschner wire is effective for distal clavicle fracture with Neer typeⅡ.It can carry out early functional exercise after operation,and is conducive to fracture healing and shoulder joint function recovery,and is worthy of clinical promotion.
