摘　　要：目的探讨锚钉结合钢丝固定治疗青少年髌骨上极套袖状撕脱骨折的临床效果。方法回顾性分析2015年11月至2019年4月收治的髌骨上极套袖状撕脱骨折8例患者的临床资料,平均年龄17.9(12~29)岁;左侧2例,右侧6例;均采用锚钉结合钢丝固定治疗。术后定期随访,摄X线片评估骨折愈合情况,并进行Lysholm评分评价膝关节功能。结果本组患者均顺利完成手术固定,术后未出现感染、锚钉脱落及撕脱骨折端移位等造成内固定物失效。所有患者均获随访,平均随访19.3(12~38)个月。术后骨折均愈合,平均愈合时间10.5(8~14)周,无膝关节活动受限,采用Lysholm评分评定膝关节功能,优5例,良2例,可1例,优良率87.5%。结论锚钉结合钢丝固定治疗青少年髌骨上极套袖状撕脱骨折效果确切,固定牢靠,可早期锻炼,功能恢复好。Objective To assess the clinical effect of anchor screw combined with steel wire in the treatment of sleeve fracture of upper patellar pole in adolescents.Methods The clinical data of 8 adolescent patients with upper patellar pole sleeve fracture admitted from November 2015 to April 2019 were analyzed retrospectively,including 5 males and 3 females,aged 12 to 29 years old with an average of 17.9.The fracture located on the left side in 2 cases and right side in 6 cases.All fractures were fixed with anchor screw combined with steel wire.Results All 8 cases were performed the internal fixation successfully.There was no infection,internal fixation failure due to anchor screw exfoliated and fracture site displacement.All cases were followed up for 12 to 38 months with an average of 19.3 months.All fractures achieved healing in 8 to 14 weeks with an average of 10.5 weeks.No restriction of knee joint movement was found.According to Lysholm score,knee joint function showed that excellence in 5 cases,good in 2 cases and fair in 1 case,and the fineness rate was 87.5%.Conclusion The internal fixation of anchor screw combined with steel wire is an effective method for upper patellar pole sleeve fracture with reliable fixation,earlier exercise and well recovery of post-operation function.
