益肾活血方联合手术治疗精索静脉曲张性不育1035例疗效分析

摘　　要：目的观察益肾活血方加手术治疗精索静脉曲张导致的弱精症的临床疗效。方法自2015年1月~2020年1月从郑州大学第一附属医院男科纳入符合标准的1947例精索静脉曲张性弱精症男性不育患者,均为肾虚血瘀型,其中1035例采用显微镜下低位精索静脉曲张结扎术加益肾活血中药,另选912例仅采用显微镜下低位精索静脉曲张结扎术设为对照组。结果术后3个月益肾活血方加手术组总有效率(81.53%)明显高于单纯手术组(59.92%)(P<0.05),益肾活血方加手术组患者精子总数﹑活力﹑精液中中性ɑ糖苷酶浓度与术前相比均有明显改善,而白细胞计数﹑附睾炎性肿块﹑阴囊坠胀疼痛与术前相比显著下降(P<0.05),精子总数、精子活力﹑精液中性ɑ糖苷酶浓度﹑白细胞计数﹑附睾炎性肿块和阴囊坠胀疼痛与单纯手术组相比也明显改善(P<0.05)。结论益肾活血方加手术通过治疗附睾炎性肿块,提高附睾功能,可以显著改善肾虚血瘀型精索静脉曲张的精子活力。Objective To observe the clinical effect of Yishenhuoxue recipe plus surgery on asthenospermia caused by varicocele.Methods 1947 male infertility patients with varicocele asthenospermia were enrolled from the andrology department of the First Affiliated Hospital of Zhengzhou University from January 2015 to January 2020,all of whom were of kidney deficiency and blood stasis type.1035 cases were treated with low ligation of varicocele under microscope plus Yishenhuoxue recipe as the observation group,and 912 cases were treated with low ligation of varicocele under microscope as the control group.Results Three months after surgery,the total effective rate of the observation group was 81.53%,which was significantly higher than that of the control group(59.92%)(P<0.05).The total sperm count,vitality and neutralɑglycosidase concentration in semen of patients in the observation group were significantly improved,while the white blood cell count,epididymal inflammatory mass and scrotal swelling pain were significantly decreased(P<0.05).Moreover,the indexes mentioned about in the observation group were significantly improved as compared with the control group(P<0.05).Conclusion Yishenhuoxue recipe plus surgery can reduce epididymal inflammatory mass and enhance epididymal functions,thus significantly improving sperm vitality of patients with varicocele due to kidney deficiency and blood stasis.
