重复捐献机采血小板对献血者血小板功能及凝血功能的影响

摘　　要：目的探讨重复捐献机采血小板对献血者血小板功能及凝血功能的影响,以评估采集血小板质量及献血者的健康状况。方法选取2018年1月~2020年5月在三明市中心血站参加无偿捐献血小板的献血者60例为研究对象,根据捐献次数分为初次捐献血者(以前没有捐献过全血或成分血者)作为对照组,连续捐献血小板1年以上超过20次作为研究组,每组各30例,分别检测两组献血者的血小板及相关参数、血栓弹力图(TEG)相关指标(R值、K值、α角、MA值)和凝血功能(PT、APTT、Fb、TT)等各项指标,并对结果进行分析。结果研究组MPV、PDW及P-LCR值分别为(10.31±0.73)fl、(11.66±1.02)%、(0.23±0.04)%,高于对照组MPV、PDW及P-LCR值[(9.53±0.58)fl、(10.43±0.92)%、(0.18±0.03)%],两组比较差异有统计学意义(P<0.05),而两组血小板计数比较,差异无统计学意义(P>0.05);研究组TEG相关指标R值、K值、α角、MA值与对照组比较,差异无统计学意义(P>0.05);研究组PT、APTT、Fb、TT等凝血功能指标与对照组相比较,差异无统计学意义(P>0.05)。结论重复捐献机采血小板的献血者血小板参数显著升高,但对血小板绝对值、血小板功能及凝血功能无明显影响,不影响献血者身体健康安全,可保障血小板质量。Objective To investigate the effect of repeated donation of machine-collected platelets on the platelet function and coagulation function of blood donors,and to evaluate the quality of platelets collected and the health of blood donors.Methods Sixty blood donors who participated in the free platelet donation at the Sanming City Central Blood Station from January 2018 to May 2020 were selected as the research subjects.According to the number of donations,they were divided into the control group(first-time blood donors those who had not donated whole blood or blood components before)and the study group(donating platelets more than 20 times continuously for more than one year),with 30 cases in each group.The platelets and related parameters,the related indicators of thromboelastography(TEG)(R value,K value,αangle,MA value),and coagulation function(PT,APTT,Fb,TT)of the two groups were detected and analyzed.Results The MPV,PDW,and P-LCR values of the study group were(10.31±0.73)fl,(11.66±1.02)%,(0.23±0.04)%,respectively,which were higher than those of the control group([9.53±0.58]fl,[10.43±0.92]%,[0.18±0.03]%),and the differences between the two groups were statistically significant(P<0.05),while there was no statistically significant difference in the platelet counts of the two groups(P>0.05).The R value,K value,αangle,and MA value of TEG related indicators in the study group did not change significantly compared with the control group,and the difference was not statistically significant(P>0.05).There were no significant differences in the PT,APTT,Fb,TT,and other coagulation function indicators between the study group and the control group(P>0.05).Conclusion The platelet parameters of blood donors who repeatedly donate platelets collected by the machine are significantly increased,but have no significant effect on the absolute value of platelets,platelet function and coagulation function,and do not affect the health and safety of blood donors,and guarantee the quality of platelets.
