经腹彩超和经阴道超声诊断异位妊娠的临床价值

摘　　要：目的探讨经腹彩超和经阴道超声诊断异位妊娠的价值。方法选取2017年9月至2019年4月我院收治的60例异位妊娠患者,所有患者均接受超声检查,先行经腹彩超检查,再行经阴道超声检查。比较两种检查方法对异位妊娠典型声像图的检出率以及诊断效能。结果经阴道超声的包块、胚芽、假孕囊、盆腔积液检出率分别为100.00%、65.00%、45.00%、76.67%,经腹彩超的包块、胚芽、假孕囊、盆腔积液检出率分别为85.00%、41.67%、30.00%、63.33%,差异有统计学意义(P<0.05)。异位妊娠彩色血流表现:以点状为主,有36例,其次为条状,有21例,半环状/环状很少,仅有3例;血流频谱表现以超低阻力型频谱为主,有28例,其次为低阻力型频谱,有15例,高阻力型频谱和舒张期血流频谱较少,分别为9例、8例。血流参数RI平均为(0.54±0.18),PI平均为(0.89±0.26)。经阴道超声对异位妊娠的敏感度、特异度分别为0.913、0.947,经腹彩超对异位妊娠的敏感度、特异度分别为0.829、0.784,经阴道超声对异位妊娠的诊断效能较经腹彩超明显提高。结论在诊断异位妊娠方面,经阴道超声较经腹彩超具有明显的优越性。Objective To explore the value of abdominal ultrasound and transvaginal ultrasonography in the diagnosis of ectopic pregnancy.Methods From September 2017 to April 2019,60 patients with ectopic pregnancy received ultrasound examination.Abdominal ultrasonography was performed first,followed by transvaginal ultrasonography.To compare the detection rate and diagnostic efficiency of the two methods for typical ultrasonography of ectopic pregnancy.Results The detection rates of mass,germ,pseudocyst and pelvic effusion by transvaginal ultrasonography were respectively100.00%,65.00%,45.00%and 76.67%,and the detection rates of mass,embryo,pseudocyst and pelvic effusion by abdominal ultrasound were respectively 85.00%,41.67%,30.00%and 63.33%,and the comparison with statistical significance(P<0.05).The color blood flow of 36 ectopic pregnancies was mainly spot-like,with 21 cases as strips and 3 cases as semi-annular/annular.The blood flow spectrum of 28 patients was of ultra-low resistance type,15 of low resistance type,9 of high resistance type,and 8 of diastolic blood flow spectrum.Blood flow parameter RI was(0.54±0.18)and PI was(0.89±0.26).The sensitivity and specificity of transvaginal ultrasound for ectopic pregnancy were respectively 0.913 and 0.947,and the sensitivity and specificity of abdominal ultrasound to ectopic pregnancy were respectively 0.829 and 0.784,and the diagnostic efficacy of transvaginal ultrasonography in ectopic pregnancy was significantly improved compared with abdominal ultrasound.Conclusion Transvaginal ultrasonography is superior to abdominal ultrasound in the diagnosis of ectopic pregnancy.
