单孔双针法腹腔镜在小儿复杂腹股沟斜疝中的应用

摘　　要：目的探讨腹腔镜单孔双针法在小儿复杂腹股沟斜疝中的应用价值。方法回顾性分析2012年1月至2014年1月,南方医科大学附属何贤纪念医院收治的复杂腹股沟斜疝120例,均为男性,分为双孔组和单孔双针组,每组60例。双孔组采用双孔法,单孔双针组则用单孔双针法。比较2组手术时间、出血量、手术并发症、住院时间及复发情况。结果2组均顺利完成手术,无输精管及血管损伤,无中转开放手术。2组手术时间比较,差异无统计学意义(P>0.05),出血量均<5 ml,均术后2 d出院。随访3~4年,中位随访时间3.5年,均无复发。结论腹腔镜单孔双针法治疗小儿复杂腹股沟斜疝安全、有效且微创。Objective To access the value of one-hole laparoscopy with diplopore suture needle for complicated indirect inguinal hernia of children.Methods A total of 120 male children with complicated indirect inguinal hernia were treated in our Department between January 2012 and January 2014.The clinical date was retrospectively.The patients were divided into 2 groups which consisted of 60 cases,respectively.Group 1 underwent a 2-port laparoscopic procedure,and Group 2 received one-hole laparoscopy with diplopore suture needle.The operation time,bleeding volume,complications,length of stay and recurrence were assessed between the groups.Results The procedures were performed successfully without serious preoperative complications,such as damage to the vas deferens and the gonadal vessels were noted.No conversion occurred in either group.All patients discharged two days after surgery.No significant difference in operation time and bleeding volume between group 1 and group 2(P>0.05).During a median follow-up period of 3.5 years(range,3 to 4 years),no postoperative hernia recurrence was encountered.Conclusion The one-hole laparoscopy with diplopore suture needle could be safe,effective,and more cosmetically appealing for the management of complicated pediatric indirect inguinal hernia.
