伴糖尿病的新型冠状病毒肺炎患者的临床特征分析

摘　　要：目的观察新型冠状病毒肺炎(COVID-19)住院患者合并糖尿病的临床特征,为临床诊治提供依据。方法回顾性分析322例COVID-19住院患者的临床资料,比较伴糖尿病COVID-19患者与非糖尿病COVID-19患者的基本特征、病情分型、炎症指标和治疗方案等方面的差别。结果322例中,伴糖尿病41例(12.73%),重型19例(20.65%)。普通型COVID-19患者的病程为(46.37±8.94)d(35~56)d,重型COVID-19患者的病程为(58.42±8.54)d(47~68 d)。伴糖尿病的重型COVID-19患者的病程与非糖尿病重型COVID-19患者比较,差别有统计学意义(P<0.05);伴糖尿病COVID-19患者铁蛋白和白细胞介素6异常的比例与非糖尿病COVID-19患者比较,差别有统计学意义(P<0.05)。伴糖尿病COVID-19患者应用抗病毒药物、降糖药物无明显不良反应,应用托珠单抗出现可逆转的白细胞下降的副作用。结论糖尿病患者感染新型冠状病毒,重型肺炎患者比例较多,且有发生炎症细胞因子风暴的风险,可能影响其病情发展。普通型COVID-19糖尿病患者预后较好,可予口服降糖药物治疗。应用免疫治疗药物时应注意药物的副作用。Objective To observe and analyze clinical characteristics of coronavirus disease 2019(COVID-19)patients complicated with diabetes mellitus.Methods 322 patients with COVID-19 were retrospectively studied and the clinical characteristics were between patients with and without diabetes.Results 41 cases(12.73%)of 322 COVID-19 patients had diabetes,19 cases(20.65%)were severe.The average disease duration was(46.37±8.94)days(35-56 days)and(58.42±8.54)days(47-68 days)for common cases and severe cases,respectively.Compared with non-diabetic patients,the difference was statistically significant.Compared with non-diabetic patients,the abnormalities of ferritin and IL-6 in COVID-19 patients with diabetes were significantly different.COVID-19 patients with diabetes had no significant side effects by using antiviral drugs or hypoglycemic drugs.Conclusions Patients with novel coronavirus infection in diabetes tend to be severe,and have the risk of cytokine release syndrome(CRS),which may affect the development of severe coronavirus pneumonia patients.The prognosis of the common cases with diabetes was benign.Oral hypoglycemic drugs can be used safely in common cases.The side effects of drugs should be vigilant when using immunotherapy drugs.
