血管超声对动脉蹼诊断价值的探讨

摘  要:目的分析动脉蹼的超声声像表现,并与血管造影结果对照,提高对超声诊断价值的认识。方法回顾性分析超声诊断的10例动脉蹼的超声表现及血管造影检查结果。结果10例动脉蹼均位于动脉后壁,其中9例发生于颈部动脉,1例发生于股动脉。超声表现为自动脉后壁突入管腔的薄片样结构,呈“肉夹馍”样改变,表面光滑,与动脉后壁成角,局部形成隐窝,可见涡流血流信号,可造成局部管腔狭窄。10例中,8例与CTA检查结果一致,1例与DSA检查结果一致,1例未行血管造影。与血管造影检查比较,血管超声诊断符合率100%。结论动脉蹼有典型的超声表现,常出现在颈部动脉后壁,超声是其筛查及临床随访的有效手段。Objective To analyze the ultrasonographic manifestations of arterial web,and compare them with the results of angiography,and to improve the understanding of its diagnostic value.Methods The ultrasonographic features and angiographic findings in 10 cases of arterial web diagnosed by ultrasonography were retrospectively analyzed.Results 9 cases of arterial web occurred in the cervical artery,1 case in the femoral artery,10 cases were located in the posterior wall of the artery;ultrasound showed that the posterior wall of the automatic pulse protruded into the lumen in a thin slice like structure,the surface was smooth,forming a recess at the angle with the posterior wall of the artery,and eddy current blood flow signals were seen at the recess,which could cause local stenosis of the artery lumen.Among them,8 cases were consistent with CTA results,1 case was consistent with DSA results,and 1 case had no angiography results.Compared with angiography,the coincidence rate of ultrasound diagnosis was 100%.Conclusion Arterial web has typical ultrasonographic findings,often found in the posterior wall of the carotid artery.Ultrasound is an effective method for screening and clinical follow-up.
