针挑联合小剂量泼尼松治疗男性抗精子抗体阳性不育症的临床观察

摘　　要：目的:观察针挑疗法联合小剂量泼尼松对AsAb阳性不育男性患者的临床疗效。方法:将2019年1月-12月收治的90例AsAb阳性不育男性患者,完全随机分为3组,治疗1组(针挑组),治疗2组(针挑加泼尼松组)和对照组(泼尼松组)各30例,治疗周期为12周,疗程结束后观察对比治疗前、治疗后三组患者的各项精液指标并对比各组疗效。结果:治疗12周后对比治疗前、后精液情况,治疗1组治愈率为30%,总有效率为75%,治疗2组治愈率为75%,总有效率为90%,对照组治愈率为15%,总有效率为45%,治疗2组总有效率明显高于对照组(P<0.01);治疗1组在精子密度、活率、前向运动精子比率和正常精子率上有明显提高(P<0.05),治疗2组在精液量、精子密度、活率、前向运动精子比率、正常精子率和顶体酶活性定量上均有明显提高(P<0.05);三组间比较,在精子密度改善情况上,治疗2组与对照组有差异(P<0.05)。结论:针挑联合小剂量泼尼松治疗男性AsAb阳性不育症疗效较好,同时可明显改善患者精液质量,值得推广。Objective:To investigate the clinical efficacy of needle-pricking acupuncture combined with low-dose prednisone in antisperm antibody(AsAb)-positive male infertility.Methods:A total of 90 AsAb-positive infertile males in our hospital between January and December of 2019 were completely randomized to three groups(n=30 each):treatment group 1(needle-pricking alone),treatment group 2(needle pricking plus prednisone)and control group(prednisone alone).The treatment course was 12 weeks.After the treatments,the three groups were compared for changes in indicators of semen quality from baseline and the clinical efficacy.Results:After 12 weeks,comparison of semen quality before and after treatment revealed that group 1 achieved a cure rate of 30%and a total response rate of 75%;these figures were 75%and 90%for group 2,15%and 45%for the control group,respectively.The total response rate in treatment group 2 was significantly higher than that in the control group(P<0.01).The treatment group 1 showed significant increase in sperm density,viability,forward motility and rate of normal sperms(P<0.05);in treatment group 2,there were significant increases in semen volume,density,viability,forward motility,rate of normal sperms,and acrosin activity(P<0.05).Among-group comparison indicated a statistical difference between the treatment group 2 and the control group regarding improvement in sperm density(P<0.05).Conclusion:Needle-pricking acupuncture combined with low-dose prednisone is effective in treating AsAb-positive male infertility and improving the semen quality in these patients,which justifies its widespread use in clinical practice.
