速效救心丸对比硝酸甘油治疗心绞痛急性发作疗效和安全性的系统评价

摘　　要：目的系统评价速效救心丸对比硝酸甘油治疗心绞痛急性发作的疗效和安全性。方法计算机检索PubMed、EMbase、Web of SCI、the Cochrane Library、中国知网(CNKI)、维普中文科技期刊全文数据库(VIP)、万方数据资源系统(WanFang Data)、中国生物医学文献服务系统(SinoMed)等数据库,纳入速效救心丸对比硝酸甘油治疗心绞痛急性发作的随机对照试验(RCT),检索时限均为建库至2019年10月30日。由两位评价者独立筛选文献、提取资料和评价纳入研究的偏倚风险后,采用RevMan 5.3软件进行Meta分析。结果纳入6项RCT,共651例心绞痛急性发作病人。Meta分析结果显示,试验组不良反应发生率低于对照组[RR=0.37,95%CI(0.20,0.69),P=0.002];两组心绞痛改善率[RR=1.00,95%CI(0.94,1.06),P=0.97]和心电图改善率[RR=0.94,95%CI(0.84,1.05),P=0.26]差异均无统计学意义。结论当前证据表明,舌下含服速效救心丸或硝酸甘油片在心绞痛急性发作的治疗方面疗效均较好,但速效救心丸不良反应更少,安全性好。Objective To evaluate the efficacy and safety of Suxiao Jiuxin pill versus nitroglycerin in the treatment of acute angina pectoris attack.Methods Randomized controlled trials(RCTs)about Suxiao Jiuxin pill versus nitroglycerin in the treatment of acute angina pectoris attack published in PubMed,EMbase,Web of SCI,the Cochrane Library,CNKI,VIP,and WanFang Data were searched since its establishment to October 30,2019.Two reviewers independently screened literature,extracted data and assessed the risk of bias of the included studies.Meta-analysis was performed by RevMan 5.3 software.Results A total of 6 RCTs involving 651 patients were included.The results of Meta-analysis showed that the adverse reactions[RR=0.37,95%CI(0.20,0.69),P=0.002]in treatment group was significantly lower than that in control group;and there was no significant difference in the improvement rate of angina[RR=1.00,95%CI(0.94,1.06),P=0.97]and the improvement rate of electrocardiogram[RR=0.94,95%CI(0.84,1.05),P=0.26]between two groups.Conclusion The current evidence shows that,the efficacy of sublingual Suxiao Jiuxin pills or nitroglycerin tablets in the treatment of acute angina pectoris attack is better,and Suxiao Jiuxin pill has fewer adverse reactions and better safety.
