摘  要:目的研究吸入基质金属蛋白酶(MMP)抑制剂对大鼠烟雾吸入肺损伤的作用。方法 86只成年雄性SD大鼠随机分为对照组(sham组)、急性烟雾损伤组(ALI组)和烟雾+MMP抑制剂治疗组(ALI+BB组)。观察各组大鼠24 h存活情况,分析大鼠血浆、BALF及肺组织中炎症相关因子表达量及蛋白渗出情况,评估肺组织通透性及组织病理学改变。结果 ALI+BB组大鼠24 h存活率为60%高于ALI组的35%(P <0.05);ALI组血浆炎性因子和BALF中蛋白含量、炎性细胞、炎性因子水平以及肺组织含水量、血管通透性、MPO、HMGB1蛋白表达均显著高于sham组(P <0.05);而ALI+BB组大鼠血浆炎性因子以及BALF中蛋白含量、炎性细胞、炎性因子水平、MPO、HMGB1蛋白表达以及肺组织含水量、血管通透性、均低于ALI组(P <0.05);与sham组相比,ALI组大鼠肺组织可见弥漫性炎性细胞浸润,肺泡内出血及大量水肿液,肺空泡腔面积显著减少;MMP抑制剂治疗则显著改善上述情况。结论 MMP抑制剂可以显著提高烟雾吸入肺损伤大鼠的存活率,减轻大鼠肺部及全身炎症反应。Objective To study the effect of inhalation of matrix metalloproteinase(MMP) inhibitor on smoke inhalation-induced acute lung injury(SI-ALI) in rats. Methods A total of 86 adult male SD rats were randomized into a sham group, an ALI group and an ALI +MMP inhibitor treatment group(ALI +BB group). The 24 h survival of the rats was observed to analyze the expression of inflammatory related factors and protein leakage in plasma, bronchoalveolar lavage fluid(BALF) and lung tissue, so as to assess the lung tissue permeability and histopathological changes. Results The 24 h survival in ALI+BB group was 60%, higher than 35% in ALI group(P < 0.05);the plasma inflammatory factors, protein content in BALF, levels of inflammatory cells and inflammatory factors,lung water content, vascular permeability, and MPO and HMGB1 protein expression in ALI group were significantly higher than those in sham group(all P < 0.05);plasma inflammatory factors and protein content in BALF, levels of inflammatory cells and inflammatory factors, and MPO and HMGB1 protein expression, as well as lung water content and vascular permeability in ALI +BB group were lower than those in ALI group(P < 0.05). Compared with sham group, ALI group showed diffuse inflammatory cell infiltration, alveolar hemorrhage and large amount of edema, and significantly reduced area of pulmonary cavity. MMP inhibitor treatment significantly improved the above situation. Conclusion MMP inhibitor can significantly improve the survival rate of rats with smoke inhalationinduced acute lung injury, and reduce the inflammatory reaction of lung and whole body in rats.
