须癣毛癣菌肉芽肿1例

摘　　要：患者女,76岁,左前臂斑块伴疼痛1年。皮肤科情况:左前臂远端皮肤见鹅蛋大小红色斑块,边缘肿胀,边界清楚,浸润感明显,有淡黄色分泌物渗出,表面皮温升高,触痛明显。皮损组织病理示:真皮内弥漫性的淋巴细胞、组织细胞、中性粒细胞及多核巨细胞呈肉芽肿样浸润;银染:可见分枝状菌丝和孢子。真菌培养分离出须癣毛癣菌。诊断:须癣毛癣菌肉芽肿。A 76-year-old female presented plaque with pain on the left forearm for one year.Physical examination revealed red plaque about goose egg size with a swollen and clear edge,obvious infiltration and yellow secretion.The lension with a high skin temlerature and tenderness.Histopathology showed diffuse inflammatory cell granulomatous infiltration with lymphocytes,histocytes,neutrophils,multinucleated giant cells.PAS and silver staining showed fungal spores and hapyae.Trichophyton mentagrophytes was isolated from the biopsy tissue.Dermatophyte granuloma caused by Trichophyton mentagrophyte was diagnosed.
