摘  要:目的探讨早产儿缺氧缺血性脑病临床诊断中磁共振波谱成像的使用价值。方法选取2018年11月—2019年10月间本院收治的早产儿合并缺氧缺血性脑病患儿30例作为观察组,另选取同期出生的正常早产儿10例作为对照组,两组患儿均给予磁共振波谱成像,对比观察两组患儿磁共振波谱成像下各项指标N-乙酰天门冬氨酸(N-acetylaspartic acid,NAA)、肌酸(Creatine,Cr)、胆碱复合物(Choline complex,Cho)及其与新生儿行为神经测定评分(Neonatal behavioral neurological score,NBNA)间关系。结果观察组患儿基底节区NAA/Cho、NAA/Cr及额叶区NAA/Cho明显低于正常早产儿,差异有统计学意义(P<0.05);观察组患儿额叶区及基底节区Cho/Cr、额叶区NAA/Cr与正常早产儿对比差异无统计学意义(P>0.05);观察组患儿基底节区NAA/Cho、NAA/Cr及额叶区NAA/Cho与NBNA评分间存在正相关关联(P<0.05)。结论磁共振波谱成像检测早产儿缺氧缺血性脑病准确率高,能在早期发现病变。Objective To explore the value of magnetic resonance spectroscopy in the clinical diagnosis of hypoxic-ischemic encephalopathy in preterm infants.Methods 30 premature infants with hypoxic-ischemic encephalopathy from November 2018 to October 2019 were selected as the observation group,10 normal preterm infants were selected as the control group.The children were given magnetic resonance spectroscopy,compared NAA,Cr,Cho.Results The NAA/Cho,NAA/Cr,and frontal NAA/Cho in the observation group were significantly lower than those in normal preterm infants,and the difference was statistically significant(P<0.05).The observation group had frontal and basal regions.Cho/Cr and NAA/Cr in the frontal region were not significantly different from those of normal preterm infants(P>0.05).NAA/Cho,NAA/Cr,and NAA/Cho and NBNA scores in the basal ganglia were observed in the observation group.There was a positive correlation between them(P<0.05).Conclusion The accuracy of using magnetic resonance wave imaging to detect hypoxic-ischemic encephalopathy in preterm infants is high.It can detect lesions at early stage.
